Operative Treatment of Lateral Malleolar Fractures by Intramedullary Screw Fixation

Abstract

Objectives: The purpose of this study is to evaluate an alternative technique of osteosynthesis of lateral malleolar fractures by intramedullary fixation.
Material and Methods: We retrospectively reviewed forty-three patients with Weber B and C type displaced lateral malleolar fractures, treated with an intramedullary, fully threaded and self-tapping screw, after closed or open reduction. The patients were examined, clinically and radiographically, after a mean follow-up period of 4 years and 1 month.
Results: The radiographic results were good (12 %) and excellent (84 %) in 96 % of patients. Functionally, there were good (21 %) and excellent (65 %) results in 85 % of patients.
No deep wound infections occurred. There were two superficial infections. Four patients had reflex sympathetic dystrophy syndrome. Screws had to be removed in 25 % of patients because of fibular malleolus pressure pain. Only two patients of this group (5 % in total) had severe complaints related to the intramedullary screw.
Conclusion: This technique requires less extensive soft tissue dissection, gives a reduction comparable to other methods, and leaves the possibility for fracture compression. In addition, there are less hardware complaints.
Although improvements of the screw are at hand, we believe that this technique is already a valuable alternative to other methods.

Zusammenfassung

Ziel: In der vorliegenden Studie wird eine alternative Technik für die Osteosynthese lateraler Malleolarfrakturen durch Markraumverschraubung evaluiert.
Material und Methoden: 43 Patienten mit Weber Typ B und C dislozierten lateralen Malleolarfrakturen, versorgt mit einer intramedullären, völlig mit Gewinde versehenen und selbstschneidenden Schraube nach geschlossener oder offener Reposition, wurden klinisch und röntgenologisch nach einer durchschnittlichen Nachbeobachtung von 4 Jahren und 1 Monat untersucht.
Ergebnisse: Die Röntgenbefunde waren gut (12 %) und ausgezeichnet (84 %) bei 96 % der Patienten. Die funktionellen Ergebnisse waren gut (21 %) und ausgezeichnet (65 %) bei 85 % der Patienten. Es gab keine tiefen Wundinfektionen. Zwei oberflächliche Infektionen wurden beobachtet. Bei 4 Patienten entwickelte sich eine Sudecksche Dystrophie.
Bei 25 % der Patienten war wegen Drückschmerzen am distalen lateralen Malleolus Entfernung der Schrauben notwendig. Nur 2 Patienten in dieser Gruppe (insgesamt 5 %) hatten ernsthafte, duch die intramedulläre Schraube verursachte Beschwerden.
Schlussfolgerung: Diese Technik erfordert eine weniger ausgedehnte Weichteildissektion, ermöglicht eine mit anderer Methoden vergleichbare Reposition und ebenfalls eine Frakturkompression. Außerdem gibt es weniger durch das Material verursachte Beschwerden.
Obwohl Verbesserungen an der Schraube geplant sind, sind wir der Meinung, dass diese Technik schon eine wertvolle Alternative für andere Methoden ist.
