Medikamentöse Compliance und Noncompliance psychiatrischer Patienten - Motive und krankheitsspezifische Zusammenhänge

Zusammenfassung

184 Patienten mit depressiven bzw. schizophrenen Störungen einer psychiatrischen Institutsambulanz wurden im Hinblick auf ihre medikamentöse Compliance/Noncompliance und ihre Motive für compliantes/noncompliantes Verhalten untersucht. Es fand sich für beide Diagnosegruppen eine bemerkenswert hohe Compliancerate und es fanden sich einige Zusammenhänge zwischen dem Therapieverhalten und demographischen bzw. klinischen Daten. Die höchste Therapietreue zeigten depressive Frauen, Patienten mit stationären Behandlungserfahrungen waren complianter als solche ohne und Noncompliance korrelierte signifikant mit zunehmender Medikamentenanzahl. Als wesentliche Grundlagen complianten Verhaltens wurden „Vertrauen zum Arzt, Wirksamkeit und Verträglichkeit der Medikamente und ausführliche und verständliche Informationen bezüglich der Medikamente” genannt. Die wichtigsten Gründe für eine ablehnende Haltung zur Medikation waren „viele Nebenwirkungen, schlechte Wirksamkeit und kein Vertrauen zum Arzt”.

Abstract

184 depressive and schizophrenic out-patients were examined with respect to their medication compliance/noncompliance. Compliance was assessed by means of a questionnaire and serum drug levels. Furthermore the patients were asked about their motives to take or refuse psychotropic drugs. The compliance was remarkably high (79.8 % resp. 71.2 %). There were some correlations between compliance/noncompliance and demographic/clinical parameters. Most compliant were depressive women. Never hospitalized individuals were less compliant than those with an in-patient experience. Compliance/noncompliance was significantly correlated with the daily number of drugs. Compliance was mostly influenced by “a good doctor-patient relationship, drugs’ efficacy, lack of side effects and sufficient information about mechanisms of action”. As main reasons for drug refusal “a lot of side effects, lack of efficacy and a poor doctor-patient relationship” were named. These results were independent of gender, age, and diagnosis.
