Gastroskopische Entfernung eines penetrierten nicht justierbaren Magenbands

Zusammenfassung

Bei Patienten mit morbider Adipositas ist die Implantation eines Magenbandes eine therapeutische Option. Neben der weitgehend verlassenen Option nichtjustierbarer Bänder wurden im letzten Jahrzehnt hauptsächlich justierbare Systeme verwendet. Eine seltene, aber potenziell schwerwiegende Komplikation stellt die Penetration in den Magen dar.
Wir berichten über eine Patientin, 58 J. 169 cm, 127 kg, BMI 44,5 kg/m². Sie präsentierte sich mit unspezifischen Oberbauchbeschwerden und der Angabe einer Gewichtszunahme von 40 kg seit 2 Jahren. 18 Jahre zuvor war ein nicht justierbares Magenband aus Teflon-Textilstreifen implantiert worden. Endoskopisch zeigt sich das Bild einer inkompletten Penetration des Bandes in den Magen mit einem verbliebenen kurzen, endoskopisch eindeutig submukös verlaufenden Anteil. Nach bildgebender Diagnostik (Röntgen Abdomen und Thorax, Sonographie), wiederum ohne Nachweis eines Peritonealkontaktes konnte das Band nach nochmaliger Mobilitätstestung gastroskopisch durchtrennt und entfernt werden, eine operative Therapie war nicht erforderlich.
Diese Therapieoption der rein gastroskopischen Entfernung eines penetrierten nicht justierbaren Magenbandes ist minimal invasiv, sie verbleibt jedoch sicher Einzelfällen ohne Peritonealkontakt des penetrierten Bandes vorbehalten.

Summary

Gastric banding is one therapeutic option in the treatment of morbid obesity. In the last decade, adjustable systems almost completely replaced formerly used nonadjustable gastric banding. One rare but potentially serious complication is intragastric band migration.
We report on a patient, a 58-year old woman, 169 cm, 127 kg, BMI 44.5 kg/m². She presented with non-specific epigastric pain and a weight gain of 40 kg during the last two years. 18 years before she had been implanted a nonadjustable gastric band formed of a teflon textile strip. Gastroscopy revealed an almost complete migration of the gastric band into the stomach, only a short section remained in a channel, that was clearly located submucosal. Also other imaging techniques (Conventional x-ray of abdomen and thorax, ultrasound) did not show any contact to the peritoneum. After repeated testing of free mobility, the gastric band could be easily cut and removed by gastroscopy, no operative therapy was necessary.
Gastroscopic removal of a migrated nonadjustable gastric band is a minimal invasive procedure, but for obvious reasons it will remain restricted to the rare cases without peritoneal contact of the migrated band.
