Zur Behandlung von ethischen Problemen und Konflikten in der Palliativmedizin

Zusammenfassung

Trotz der umfassenden Möglichkeiten der Palliativmedizin zur Symptomkontrolle und Leidensminderung werden die an der Betreuung sterbenskranker Menschen Beteiligten häufig mit existenziellen Sinnfragen, ethischen Problemen und Konflikten konfrontiert, in denen sich im Spannungsfeld zwischen Selbstbestimmung und medizinethischen Prinzipien unterschiedliche Interessen zwischen Patient und Therapeuten manifestieren. Für den Umgang mit Konflikten in der Palliativsituation sind nicht nur Kenntnisse der verschiedenen Dimensionen ethischer Probleme notwendig, sondern auch ein am Konfliktmodell der Sozialpsychologie orientiertes systematisches Vorgehen, in dem sieben Schritte unterschieden werden können: Identifikation des ethischen Problems - Akzeptanz - Analyse des ethischen Konflikts - Interessenbestimmung - Wertanalyse - Konfliktlösung - Konsolidierung. Am Beispiel einer Patientin, die in hilflosem und desorientiertem Zustand auf die Palliativstation eingewiesen wurde, werden die verschiedenen Schritte in einer Konfrontation mit ethischen Konfliktsituationen exemplarisch dargestellt.

Abstract

Though palliative care can contribute substantially to relieve suffering even in extreme situations carers are often confronted with ethical problems and conflicts, which may arise when patients' existential demands for self-determination diverge from ethical principles in medicine or the therapist's personal values. Adequate handling of the problem situation requires competent knowledge of the different dimensions of ethics, behavioural skills and sensitivity for emerging conflicts. It is suggested that the orientation on a social-psychologically based conflict model can be useful for conflict management. Steps to be considered should include identification of the problem, acceptance of the conflict, analysis of conflict dimensions, determination of interests, conflict resolution and consolidation. A case report is presented to demonstrate the different stages in ethical conflict situations.
