Abstract
A small number of cavernous liver hemangiomas in infants cause serious symptoms, requiring active treatment. We report two newborns with giant liver hemangiomas, treated by intra-arterial embolization. The babies presented at 2 - 8 days after birth with tachypnoea and cardiac dilation. A giant liver hemangioma located in the right liver lobe in one infant and in the left liver lobe in the other was found at ultrasonography and computed tomography. Dilated liver veins indicated abnormal shunting of the blood through the hemangiomas. Because of progress of symptoms superselective embolization of the arteries feeding the hemangiomas and arising from the celiac trunk was performed with a mixture of Lipoidol and Histoacryl. A decrease of tachypnoea and of heart volume was noted after embolization. In one infant surgery was necessary due to gastrointestinal bleeding.
The intra-arterial embolization is a valuable method for the treatment of newborns with symptomatic cavernous liver hemangiomas.

Un petit nombre d'hémangiomes caverneux du foie chez l'enfant entraîne des symptômes qui nécessitent un traitement actif. Nous rapportons le cas de 2 enfants avec un hémangiome hépatique géant traité par embolisation intra-artérielle. Les enfants se présentaient à 2 et 8 jours après la naissance avec une tachypnée et une dilatation cardiaque. Un hémangiome géant du foie localisé dans le foie droit d'un enfant et dans le foie gauche de l'autre était trouvé à l'échographie et au scanner. Les veines dilatées du foie indiquaient un shunt anormal à travers l'hémangiome. Devant l'augmentation des symptômes, une embolisation hyper-élective des artères nourrissant l'hémangiome qui naissaient du tronc cœliaque était réalisée avec un mélange de Lipiodol et d'Histoacryl. Une diminution de la tachypnée et du volume du foie était notée après l'embolisation.
Chez 1 enfant, la chirurgie a été nécessaire devant des hémorragies gastro-intestinales. L'embolisation intra-artérielle est une méthode valable pour traiter les nouveau-nés avec des hémangiomes hépatiques symptomatiques.
Resumen
Una pequeña parte de los hemangiomas cavernosos del hígado en niños pequeños pueden causar síntomas graves que necesitan tratamiento activo. Presentamos dos recién nacidos con hemangiomas hepáticos gigantes tratados por embolización intra-arterial. Los niños se presentaron entre 2 y 8 días tras el nacimiento con taquipnea y cardiomegalia. Se hallaron mediante ultrasonorografía y TAC hemangiomas gigantes del hígado localizados en el lóbulo derecho en un niño y en el izquierdo en el otro. Las venas hepáticas dilatadas indicaban cortocircuíto anormal de la sangre a través de los hemangiomas. A causa de la progresión de los síntomas se realizó embolización supraselectiva desde el tronco celiaco de las arterias nutritivas de los hemangiomas con una mezcla de Lipiodol e Histoacryl. Disminuyó la taquipnea y la cardiomegalia después de la embolización. En un niño fue necesaria la cirugía por hemorragia digestiva.
La embolización intra-arterial es un método valioso de tratamiento en recién nacidos con hemangiomas cavernosos del hígado sintomáticos.
Zusammenfassung
Eine kleine Zahl von kavernösen Leberhämangiomen im Kindesalter verursacht schwerwiegende Symptome, die eine aktive Behandlung erfordern.
Es wird über zwei Neugeborene mit riesenhaften Leberhämangiomen berichtet, die mittels intraarterieller Embolisation behandelt wurden. Die Babys wurden 2 - 8 Tage nach der Geburt mit Tachypnoe und Herzdilatation vorgestellt. Bei einem Kind war das übergroße Leberhämangiom im rechten, bei dem anderen im linken Leberlappen lokalisiert. Die Diagnose wurde mittels Ultraschall und Computertomographie gestellt. Dilatierte Lebervenen wiesen auf einen pathologischen Shunt innerhalb der Hämangiome hin. Da die Symptome zunahmen, wurde eine superselektive Embolisierung der zuführenden Arterien der Hämangiome, die vom Truncus coeliacus ausgingen, durchgeführt und eine Mischung von Lipiodol und Histoacryl in die Arterien appliziert. Es kam zu einer Abnahme der Tachypnoe und des Herzvolumens. Bei einem Kind musste jedoch wegen einer gastrointestinalen Blutung ein operativer Eingriff zusätzlich vorgenommen werden.
Schlussfolgerung
Die intraarterielle Embolisation ist eine wirksame Methode in der Behandlung symptomatischer, progressiver, großer, kavernöser Leberhämangiome im Neugeborenenalter.
