Tubo-Ovarian Abscess after Colonic Vaginoplasty for High Cloacal Anomaly in a 13-Year-Old Girl

Abstract

Recently a few articles have been published concerning the long-term follow-up of vaginoplasty of cloaca. However, no postoperative evaluation has been fully described and, in particular, the late complications are still unknown. We report a case of tuboovarian abscess after colonic vaginoplasty for high cloacal anomaly in a 13-year-old girl. She required a left salpingo-oophorectomy and postoperatively showed regular menstruation. Therefore we stress that tubo-ovarian abscess is one of the important late complications after colonic vaginoplasty for high cloacal anomaly.

Résumé

Récemment, quelques articles ont été publiés concernant le suivi à long terme des vagino-plasties pour cloaque. Toutefois, aucune évaluation post-opératoire complète n'a été publiée et, en particulier, les complications tardives sont encore inconnues. Nous rapportons le cas d'un abcès tubo-ovarien après vaginoplastie colique pour une anomalie cloacale haute chez une fille de 13 ans. Cet abcès a nécessité une salpingectomie gauche. En post-opératoire, l'enfant était réglée régulièrement. Nous concluons que l'abcès tubo-ovarien est l'une des complications tardives importantes après vaginoplastie colique pour anomalie cloacale haute.

Resumen

Recientemente se han publicado algunos artículos sobre el seguimiento a largo plazo de vaginoplastias por cloaca. Sin embargo no se ha descrito evaluación postoperatoria completa alguna y en particular desconocemos las complicaciones tardías. Presentamos un caso de absceso tuboovárico tras vaginoplastia con colon por anomalía cloacal alta en una niña de 13 años. Requirió una salpingoooforectomía izquierda y recuperó mestruaciones normales en el postoperatorio. Destacamos que los abscesos tuboováricos pueden ser una de las complicaciones tardías importantes de la vaginoplastia con colon por anomalías cloacales altas.

Zusammenfassung

Vor kurzem sind einige Arbeiten über Langzeitnachuntersuchungen bei Kindern mit Vaginoplastik nach Korrektur einer Kloakenfehlbildung publiziert worden. Einzelheiten über Spätkomplikationen sind jedoch weitgehend unbekannt und Langzeituntersuchungen fehlen. In der vorliegenden Arbeit wird ein Mädchen mit einem Tuboovarialabszess nach Kolovaginoplastik vorgestellt. Bei dem jetzt 13-jährigen Mädchen war eine hohe Kloakenfehlbildung operiert und die Vagina aus einem Kolonsegment rekonstruiert worden. Sie entwickelte einen tubovariellen Abszess, so dass eine linksseitige Salpingo-Oophorektomie notwendig wurde. Postoperativ hatte sie regelmäßige Menstruationen.

Schlussfolgerungen

Ein tuboovarieller Abszess ist eine der wichtigen Spätkomplikationen einer späten Koloninterposition zur Vaginalrekonstruktion bei hohen Kloakenfehlbildungen. Er kann die Folge einer nicht ganz vollständigen Durchgängigkeit der Tube sein, weshalb bei der primären Operation der Kloakenfehlbildung immer auf die Durchgängigkeit der Tuben geachtet werden sollte.
