Zusammenfassung
Erste Erfahrungen mit der intramedullären Verriegelungsnagelung metaphysärer Humerusfrakturen werden analysiert. Zur Verfügung stand einerseits ein konventioneller Humerusmarknagel (UHN), welcher mit einer an der Nagelbasis winkelstabil verblockten Spiralklinge vor allem bei der Kombination aus ipsilateraler diaphysärer und metaphysärer Humerusfraktur eine bessere Stabilisierung bewirken soll. Andererseits dient ein neuer kurzer Nagel, der proximale Humerusnagel (PHN), mittels der gleichen Spiralklingenverriegelung der möglichst geschlossenen Versorgung metaphysärer Humerusfrakturen. Die Implantate kamen bei 52 Patienten zur Anwendung. Deutlich wurde, dass der Umgang und Einsatz beider Implantate über den antegraden Zugang technisch komplikationsarm ist. Bei 11A-Frakturen reichen kleine Stichinzisionen zur Implantation aus, bei 11B-Frakturen kann eine zusätzliche Fadenzuggurtung Tuberkulafragment an der Spiralklingenbasis fixieren. Bei 11C-Frakturen werden größere Zugänge notwendig, um die exakte Reposition und ggf. zusätzliche Schraubenimplantationen zu ermöglichen. In zwei Fällen musste bei Humeruskopfnekrosen im weiteren Verlauf der hemiendoprothetische Ersatz erfolgen, alle anderen Fälle erforderten bis zum jetzigen Zeitpunkt keinen Verfahrenswechsel. Das funktionelle Ergebnis deutet darauf hin, dass A- und B-Frakturen von diesem Verfahren sehr profitieren, bei C-Frakturen, ähnlich anderen operativen Verfahren, eher mit Bewegungsdefiziten zu rechnen ist. Zusammenfassend gestaltet sich die intramedulläre Nagelung metaphysärer Humerusfrakturen als wertvolle Alternative gegenüber den traditionellen operativen Verfahren.
Abstract
Early experience with interlocked intramedullary nailing of metaphyseal humeral fractures will be analysed. A conventional humeral nail (UHN) was available, which has been combined with an angle stable spiral blade for stabilising the combination of ipsilateral diaphyseal and metaphyseal humeral fractures. Furthermore for closed treatment of metaphyseal humeral fractures a new short nail, the proximal humeral nail (PHN), has been used. These implants had been adopted in 52 patients. It was clear, that handling and performance with both implants through the antegrade approach remained easy. In type 11A-fractures small stitch incisions were sufficient, in type 11B-fractures additional suture-tension-bands could stabilise tubercular fragments. In type 11C-fractures larger approaches were necessary, to gain exact reduction and possible additional screw fixation. In two patients humeral head necrosis lead to prosthetic replacement, in all other cases until now no change of procedure has been necessary. The functional results show, that type A- and B-fractures will profit very well from nailing, in type C-fractures functional deficits are more likely. In conclusion intramedullary nailing of metaphyseal fractures is a valuable alternative to traditional operative methods.
