Verletzungen des oberen Sprunggelenkes bei Unterschenkelfrakturen

Zusammenfassung

Ziel dieser retrospektiven Untersuchung ist es, den Zusammenhang zwischen Sprunggelenksbeteiligung, insbesondere dem Auftreten eines Volkmann-Dreiecks und Unterschenkelfrakturen nachzuweisen. Zur Auswertung kamen dabei 162 von 196 Unterschenkelfrakturen, die in der Zeit vom 2. 1. 1996 bis 27. 3. 1998 an der unfallchirurgischen Abteilung des Krankenhauses Zell am See operativ versorgt wurden. Zum Auftreten eines Volkmann-Dreiecks kam es insgesamt in 23 Fällen (14,2 %). 92 Patienten hatten eine spiralförmige Fraktur, davon 19 Patienten (20,6 %) mit Auftreten eines Volkmann-Dreiecks, welches bei 12 Patienten (13,0 %) von 92 osteosynthetisch mitversorgt werden musste. Zur Vermeidung schlechter funktioneller Ergebnisse und einer unfallbedingten Früharthrose im oberen Sprunggelenk soll darauf hingewiesen werden, dass bei der Diagnose einer Unterschenkelfraktur, insbesondere Spiralbruch, die Beurteilung des oberen Sprunggelenkes nicht vernachlässigt werden darf. Spätestens intraoperativ sollte unter Röntgenkontrolle das obere Sprunggelenk mitbeurteilt und gegebenenfalls mitversorgt werden.

Abstract

The aim of this retrospective investigation was to demonstrate the association between lower leg fractures and ankle joint involvement, particularly the occurrence of a Volkmann's triangle. 162 lower leg fractures were evaluated from a total of 196 cases surgically treated in the Traumatology Department of the Zell am See Hospital, Austria, between 2. 1. 1996 and 27. 3. 1998. A Volkmann's triangle occurred in 23 cases (14,2 %). 92 patients had a spiral fracture; 19 (20,6 %) of these had a Volkmann's triangle and 12 (13,0 %) of 92 required osteosynthesis. To avoid poor functional outcome and trauma-related early arthrosis of the ankle joint, we recommend to examine the ankle joint when diagnosing lower leg fractures, particularly spiral fractures. At the latest during surgery, the ankle joint should be x-rayed and, if necessary, treated concurrently.
