Die Pathogenese der Zwangsstörung

Abstract
Die Zwangsstörung stellt eine heterogene Erkrankung dar. Neben psychologischen spielen verschiedene neurobiologische Faktoren bei der Pathogenese der Zwangserkrankung eine entscheidende Rolle. In diesem Artikel sollen die verschiedenen pathophysiologischen Modelle vorgestellt werden. Dabei wird darauf eingegangen, wie diese unterschiedlichen Modelle zu den differenzierten klinischen Erkrankungsbildern und Untergruppen beitragen können. Die heuristischen Modelle versuchen die Zwangsspektrumserkrankungen wie die Tourette- und Ticstörung, die Trichotillomanie und die Körperdysmorphophobie mit einzubeziehen und zu erklären, da sie wahrscheinlich eine gemeinsame neurobiologische Basis besitzen. Ein wichtiges Ziel zukünftiger Forschungsvorhaben ist die Identifizierung neurobiologischer Variablen, die ein Ansprechen auf die Verhaltenstherapie vorhersagen können. So kann eine differenzierte Subtypisierung der heterogenen Gruppe von Zwangspatienten erfolgen, die unmittelbare Auswirkung auf entsprechend differenziellere und spezifischere Therapie-Strategien hat.
