Abstract
Intestinal bacterial proliferation is an important aspect of gastrointestinal injury in neonatal necrotizing enterocolitis (NEC). In the present investigation, we examined the protective action of oral supplementation with Saccharomyces boulardii (S. boulardii), non-pathogen probiotic yeast, against hypoxia-reoxygenation (H/R)-induced NEC in young mice. Young mice were divided into three groups; Group 1 mice (untreated) were subjected only to hypoxia-reoxygenation; Group 2 mice were subjected to hypoxia-reoxygenation and were then given lyophilized S. boulardii (10 mg suspended in 0.5 ml saline) twice a day by orogastric intubation for 10 days. Group 3 mice served as controls. Hypoxia was induced by placing young mice in a 100 % CO2 chamber for 5 min. After hypoxia, the young mice were reoxygenated for 10 min with 100 % oxygen. We examined the intestinal lesions by light microscopy and measured intestinal generation of PAF and TNF-alpha in the H/R-induced model of NEC. In the probiotic group, NEC-induced intestinal tissue damage was greatly attenuated, with necrosis partially limited to the mucosa. Both intestinal tissue PAF and TNF-alpha concentrations were significantly higher in the untreated group than in controls (p < 0.001). S. boulardii-supplemented young mice showed a significant decrease in intestinal generation of PAF compared with untreated young mice (p < 0.05). On the other hand, no significant difference was observed in the intestinal concentration of TNF-alpha between untreated and probiotic groups (p > 0.05). The present study suggests that hypoxia/reoxygenation plays an important role in the pathogenesis of NEC and supports hypothesis that especially PAF and TNF-alpha are involved in the pathophysiological mechanism of H/R-induced NEC. This study also demonstrates that dietary supplementation with S. boulardii ameliorates the histologic evidence of H/R-induced intestinal injury. Based on these findings, the beneficial effects of probiotic S. boulardii in this model of NEC are mediated via mechanisms inhibiting intestinal proinflammatory mediator release.

La prolifération bactérienne intestinale est un composant important en cas de lésions digestives par entérocolite nécrosante néonatale (ENN). Nous avons étudié l'action protectrice d'un apport alimentaire en Saccharomyces boulardii, une levure probiotique non pathogène, pour lutter contre l'hypoxie-réoxygénation induite par l'ENN chez des jeunes souris. Trois groupes de souris était étudiés. Groupe 1 (sans traitement): hypoxie-réoxygénation; Groupe 2: traitement par lyophilised S. boularidii (10 mg dans 0,5 ml sérum physiologique) deux fois par jour pendant dix jours via un tube orogastrique; Groupe 3: témoins. L'hypoxie était induite en plaçant les souris dans une chambre sous atmosphère à 100 p.100 CO2 pendant 5 minutes. Après induction de l'hypoxie les souris étaient réoxygénées pendant 10 minutes avec de l'oxygène à 100 p.100. Nous avons examiné les lésions intestinales sous microscopie optique et mesuré la production intestinale de PAF et de TNFa. Dans le groupe probiotique, les lésions intestinales induites par la ENN étaient nettement atténuées, avec une nécrose limitée partiellement à la muqueuse. Les concentrations du tissu intestinal en PAF et en TNFa étaient plus élevées dans le groupe sans traitement que dans le groupe témoin (p < 0,001). L'apport oral de S. boulardii a permis une diminution significative de la production intestinale de PAF comparée au groupe sans traitement (p < 0,05). Il n'y avait pas de différence significative entre la concentration en TNFa des groupes sans traitement et probiotique (p > 0,05). Cette étude suggère que l'hypoxie/réoxygénation joue un rôle important dans la pathogénèse de l'ENN et que le PAF et le TNFa participent au mécanisme physiopathologique de l'hypoxie/réoxygénation induite par l'ENN. Par ailleurs, cette étude montre qu'un apport quotidien en S. Boulardii améliore l'aspet histologique des lésions intestinales induites par l'hypoxie-reéoxygénation. Sur la base de ces observations, on peut conclure à l'effet bénéfique du probiotique S. boulardii dans ce modèle d'ENN et que cet effet résulte d'une inhibition de la libération de médiateurs proinflammatoires intestinaux.
Resumen
La proliferación de bacterias intestinales es un componente importante de la lesión gastrointestinal en la enteritis necrotizante (NEC). En esta investigación examinamos la acción protectora de suplemento oral con Saccharomyces boulardii, hongo probiótico no patógeno, contra la enterocolitis necrotizante inducida por hipoxia-reoxigenación en jóvenes ratones. Los ratones fueron divididos en tres grupos: Grupo 1 (no tratados) que sufrieron hipoxia-reoxigenación. Grupo 2, tratados con S. boulardii liofilizado (10 mg suspendidos en 0.5 ml de suero) 2 veces al día por intubación orogástrica durante 10 días. El grupo 3 fueron ratones controles. Se indujo hipoxia colocando a los ratones en una cámara con anhídrido carbónico al 100 % durante 5 minutos. Tras la hipoxia los ratones fueron reoxigenados durante 10 minutos con oxígeno al 100 %. Examinamos las lesiones intestinales con microscopía convencional y medimos la generación intestinal PAF y TNF en el modelo de NEC inducido por H/R. En el grupo con probióticos, el daño del tejido intestinal inducido por NEC estaba muy atenuado con necrosis parcial limitada a la mucosa. Las concentraciones intestinales de PAF y TNF-alfa eran significativamente mayores en los animales no tratados que en los controles (p < 0,001). Los ratones suplementados con S. boulardii mostraron una disminución significativa de la generación intestinal de PAF en comparación con ratones no tratados (p < 0,05). Por otra parte no hubo diferencias significativas en la concentración intestinal de TNF-α entre los grupos no tratados y tratados con probióticos (p > 0,05). Esta investigación sugiere que la hipoxia/reoxigenación juega un papel importante en la patogenia de la NEC y apoya que especialmente PAF y TNF-α estén implicados en el mecanismo fisiopatológico de la NEC inducida por H/R. También demuestra que la suplementación en la dieta con S. boulardii mejora la evidencia histológica de lesión intestinal inducida por H/R. Basados en estos hallazgos creemos que lo efectos beneficiosos del probiótico S. boullardii en este modelo de NEC se median por mecanismos que inhiben la liberación intestinal de mediadores proinflamatorios.
Zusammenfassung
Bei der nekrotisierenden Enterokolitis des Neugeborenen ist die bakterielle Proliferation ein wichtiger Faktor für die gastrointestinale Schädigung. In der vorliegenden Untersuchung wird die Protektion des Magen-Darm-Traktes durch die Gabe von Saccharomyces boulardii (S. boulardii) als nicht pathogenes, probiotisches Medikament gegen die Hypoxie-Reoxygenierung (H/R-induzierte NEC) bei jungen Mäusen überprüft.
Methodik
Die jungen Mäuse wurden in 3 Gruppen unterteilt. Gruppe 1: Nicht behandelte Mäuse wurden nur einer Hypoxie und Reoxygenierung unterzogen. In der Gruppe 2 erhielten die Tiere lyophilisierte S. boulardii-Keime (10 mg aufgelöst in 0,5 ml physiologischer Kochsalzlösung) zweimal täglich orogastral für eine Dauer von 10 Tagen. Die Gruppe 3 der Mäuse diente als Kontrollgruppe. Die Hypoxie wurde ausgelöst bei den jungen Tieren in einer 100 %igen C02-Kammer für 5 Minuten. Nach der Hypoxie wurden die Tiere für 10 Minuten mit 100 %igem Sauerstoff reoxygeniert. Die Schädigung des Intestinaltraktes wurde lichtmikroskopisch untersucht und es wurde die Schädigung durch freie Radikale und Entzündungsfaktoren, einschließlich Tumornekrosefaktoren (TNF), Thrombozyten aktivierenden Faktoren (platelet activating factor) PAF, Leukotrinen (LT) und TNF-alpha untersucht.
Ergebnisse
In der probiotischen Gruppe war die intestinale Schädigung geringer, die Nekrose beschränkte sich nur auf die Mukosa. Die Gewebs-PAF- und TNF-alpha-Konzentrationen waren in der nicht behandelten Gruppe wesentlich höher als in der Kontrollgruppe. Die mit S. boulardii behandelten Tiere zeigten eine signifikant herabgesetzte PAF gegenüber den nicht behandelten Mäusen (p < 0,05). Auf der anderen Seite fanden sich keine signifikanten Unterschiede hinsichtlich TNF-alpha zwischen nicht behandelten und probiotisch behandelten Tieren (p < 0,05).
Schlussfolgerungen.
Die vorliegende Studie zeigt, dass Hypoxie und Reoxygenierung eine wichtige Rolle in der Pathogenese der NEC darstellen. Darüber hinaus zeigen die Untersuchungen, dass PAF und TNF-alpha in dem pathophysiologischen Prozess der H/R-induzierten NEC eine Rolle spielen. Die Studie zeigt weiterhin, dass die Gabe von S. boulardii offensichtlich einen positiven Einfluss auf die Histologie und den durch die NEC entstandenen Schaden hat. Die positiven Effekte von S. boulardii bei diesem NEC-Modell basieren auf einem Hemm-Mechanismus entzündungsstimulierender Mediatoren.
