Congenital Left-Sided Bochdalek Diaphragmatic Hernia. Thoracoscopic Repair - Case Report

Abstract

The authors report the case of a 12-year-old girl who presented with a left-sided diaphragmatic hernia that was diagnosed after a pulmonary infection. CT scan confirmed a posterolateral diaphragmatic Bochdalek defect.

The child underwent a thoracoscopic repair of the defect in 1997. The herniated contents included spleen, stomach, and intestine. The child was discharged from hospital the day after surgery. She has been followed up and is clinical and radiologically well.

Résumé

Les auteurs présentent le cas clinique d'une jeune fille de douze ans avec une hernie diaphragmatique à gauche, découverte à la suite d'une pneumonie. Le scanner a confirmé un défaut diaphragmatique postérolatéral de Bochdalek. Cette jeune fille a subi une phrénorraphie thoracoscopique en 1997: nous avons introduit trois trocarts (un de 10 mm de diamètre et deux de 5 mm) et utilisé des pressions intrapleurales d'environ 7 mm Hg. Le contenu herniaire incluait la rate, l'estomac et une portion du grêle. Cette jeune fille est sortie de l'hôpital le jour suivant à l'opération. Suivie en consultations externes, l'évolution clinique et l'aspect radiologique ont été satisfaisants.

Resumen

Los autores presentan el caso de una niña de 12 años que portaba una hernia diafragmática izquierda diagnosticada tras una infección pulmonar. Un TAC confirmó el defecto diafragmático posterolateral de Bochdalek.

La niña fue operada toracoscópicamete en 1997, reduciéndose el contenido que incluía el bazo, el estómago y el intestino. Fue dada de alta al día siguiente tras la cirugía y está clínica y radiológicamente bien desde entonces.

Zusammenfassung

Es wird ein 12-jähriges Mädchen mit einer Zwerchfellhernie, die erst nach einer Lungenentzündung diagnostiziert wurde, vorgestellt. Das CT bestätigte einen postero-lateralen Bochdalekschen Defekt. Es wurde ein thorakoskopischer Verschluss des Defektes durchgeführt. Dabei fanden sich in der Hernie Milz, Magen und Darm. Bereits am Tage nach der Operation wurde das Kind nach Hause entlassen. Die klinische Entwicklung und radiologischen Kontrollen waren bisher befriedigend.
