Arbeitstherapie in der Behandlung und Rehabilitation psychiatrischer Patienten

Zusammenfassung

Arbeitstherapie ist ein soziotherapeutisches Behandlungsverfahren, in dem Arbeit als Mittel eingesetzt wird, um therapeutische und rehabilitative Ziele zu erreichen. Für die Wirkung sind Lernprozesse von großer Bedeutung. Die praktische Entwicklung geht zunehmend in Richtung ambulanter Angebote. Über eine differenzielle Indikationsstellung sowie Frequenz und Dauer der Maßnahme liegen wenige empirische Ergebnisse vor. Als therapeutische Effekte sind bei schizophrenen Patienten eine Verbesserung der Arbeitsfähigkeit und beruflichen Belastbarkeit sowie eine Reduktion von Rehospitalisierungen durch kontrollierte Studien ausreichend gesichert. In Bezug auf eine berufliche Wiedereingliederung scheint der supported employment Ansatz effektiver zu sein. Die Befunde zur Verbesserung der psychischen Symptomatik sind widersprüchlich. Die klinische Bedeutung der Arbeitstherapie liegt v. a. darin, dass sie einen frühen und niederschwelligen Einstieg in die Arbeitsrehabilitation ermöglicht und sehr flexibel einsetzbar ist. Für viele Patienten hat sie eine Start- und Verteilerfunktion. Wesentlich für ihren Erfolg ist die Kooperation mit den anderen Diensten und Einrichtungen in der psychiatrischen Arbeitsrehabilitation.

Abstract

Occupational therapy is a psychosocial intervention using work as a means of reaching individual goals in psychiatric treatment and rehabilitation. Therapeutic effects mainly base upon social and instrumental learning processes. Increasingly more occupational therapy programmes are provided as outpatient programmes. Little is known about the frequency optimum and duration of the intervention. Controlled studies have proved that vocational rehabilitation programmes for schizophrenic patients lead to a reduction of rehospitalisation rates and improve their capacity to work. Supported employment programmes seem to be more effective concerning reintegration into competitive employment. Regarding the impact on psychopathological symptoms the results are inconsistent. Occupational therapy provides the opportunity of an early, flexible and low-threshold start of vocational rehabilitation. The cooperation with outpatient services in the community is crucial to ensure success in vocational rehabilitation.
