Abstract
The objective of this study was to determine treatment strategies for children admitted with pleural empyema. We reviewed the medical records of 93 consecutive patients with pleural empyema who presented in the last three years. Mean age was 3.8 years (range 3.5 months to 14 years) and the male to female ratio was 52 : 41. The left side was involved in 51 (54.8 %) cases and there was only one case with bilateral effusion. After the chest X-ray and analysis of pus obtained by thoracocentesis, the initial treatment modality was chest tube drainage in 81 (87 %) patients. Children required chest tube drainage for an average of 11.9 ± 4.8 days. Out of the 81 children, 6 needed thoracotomy due to organized pleural fluid and another 4, who developed loculated pleural effusions, were treated with intrapleural urokinase (UK) administration. Twelve children, who had a delayed presentation with organized loculated pleural fluid and pleural thickening, underwent decortication and one, who was found to have a foreign body, found during surgery, needed an upper lobe resection. Medical management with adequate chest tube drainage and appropriate antibiotics has resulted in full resolution in the majority of patients with empyema. In cases of loculated pleural effusions identified at the early stages of the chest tube drainage, intrapleural UK administration was found to be a safe and efficient treatment modality. Thoracotomy should be reserved for late and organized empyema cases.

L'objectif de cette étude est de déterminer les stratégies thérapeutiques appropriées chez les enfants présentant un empyème pleural à partir d'une analyse rétrospective des dossiers de 93 patients consécutifs au cours des 3 dernières années. L'âge moyen des patients était de 3,8 ans (extrêmes 3,5 mois - 14 ans). Il y avait 52 garçons et 41 filles. Le côté gauche était atteint dans 51 des cas (54,8 %) sans cas bilatéral. Après l'analyse des radiographies et du prélèvement par thoracocentèse, le traitement initial était un drainage percutané chez 81 patients (87 %). La durée moyenne du drainage était de 11,9 ± 4,8 jours. Une thoracotomie était réalisée chez 6 patients en raison d'une collection organisée du liquide pleural. Chez 4 autres un épanchement organisé était traité par administration intrapleurale d'urokinase. Chez 12 patients de présentation tardive, un épanchement cloisonné avec épaississement des parois a nécessité une décortication avec découverte d'un corps étranger menant à une lobectomie dans un cas.
La prise en charge médicale avec drainage thoracique et antibiothérapie a permis la résolution totale de l'empyème chez la majorité des patients. En cas d'épanchement cloisonné découvert au cours des premiers jours de drainage, l'administration intrapleurale d'urokinase est une méthode efficace et sans danger. Le recours à la thoracotomie ne serait nécessaire qu'en cas d'épanchement organisé ou découvert tardivement.

El objetivo de este estudio fue determinar las estrategias del tratamiento para niños ingresados por empiema pleural. Revisamos las historias de 93 pacientes consecutivos con empiema que se presentaron en los últimos 3 años. La edad media fue de 3.8 años (rango 3.5 meses a 14 años) y la relación varón/mujer fue de 52 : 41. El lado izquierdo fue el más frecuentemente afectado en 51 casos (54.8 %) y solamente hubo un caso bilateral. Tras la radiografía de tórax y el análisis del pus obtenido por toracentesis la modalidad inicial de tratamiento fue el drenaje con tubo pleural en 81 (87 %) de los pacientes. El tubo se mantuvo durante una media de 11.9 ± 4.8 días. De los 81 niños, 6 necesitaron toracotomía debido a una organización del derrame pleural y otros 4, que desarrollaron derrames loculados, fueron tratados con administración intrapleural de uroquinasa. Doce pacientes que se presentaron tardíamente con lóculos de líquido pleural y pleura engrosada sufrieron decorticación y uno que tenía un cuerpo extraño hallado durante la cirugía necesitó una resección del lóbulo superior. El tratamiento médico con drenaje pleural y antibióticos apropiados consiguió la resolución total en la mayoría de los pacientes con empiema. En casos de derrames loculados identificados en etapas precoces del tratamiento la adición de uroquinasa fue eficaz y segura. La toracotomía debe ser reservada para los empiemas tardíos y organizados.

Zielsetzung: Ziel der Arbeit ist es, die therapeutischen Richtlinien in der Behandlung eines Pleuraempyems festzulegen.
Material und Methodik: Die Krankengeschichten von 93 konsekutiv behandelten Patienten mit Pleuraempyem der letzten 3 Jahre wurden überprüft. Das Durchschnittsalter lag bei 3,8 Jahren (3,5 Monate bis 14 Jahre), die Geschlechterverteilung männlich zu weiblich betrug 52 : 41. Die linke Seite war bei 51 Kindern (54,8 %) betroffen, nur bei einem Patienten waren beide Seiten infiziert.
Ergebnisse: Nach der Röntgendiagnose und Pleurapunktion wurde bei 81 Kindern (87 %) eine Pleuradrainage gelegt. Die Kinder benötigten die Drainage durchschnittlich 11,9 ± 4,8 Tage. Von den 81 Patienten mussten 6 einer Thorakotomie unterzogen werden, da es zu einer Organisation des Empyems gekommen war. Weitere vier konnten mit interpleuraler Urokinase behandelt werden. In 12 Fällen, die verspätet zur Vorstellung kamen, musste eine Dekortikation durchgeführt werden, wobei bei einem Kind aufgrund eines während der Operation gefundenen Fremdkörpers eine Lungenoberlappenresektion notwendig wurde.
Schlussfolgerungen: Die konservative Behandlung mit ausreichender Drainage und adäquater Antibiotikagabe ist die Therapie der Wahl bei der Mehrzahl der Kinder mit Pleuraempyem. Bei lokalisierten Empyemen, die einer Drainage nur noch schwer zugänglich sind, oder bei intrapleuralen Membranbildungen, kann frühzeitig mit einer Urokinasebehandlung begonnen werden. Die Thorakotomie ist Fällen vorbehalten, die sehr spät zur Vorstellung kommen und bei denen das Empyem bereits vollständig organisiert ist. Eine thorakoskopische Therapie kann in der Frühphase von Nutzen sein, ihr Wert muss jedoch mit einer randomisierten Studie erst bewiesen werden.
