Abstract
Localised traumatic abdominal hernias are rare. Most such hernias are due to the direct impact of the handlebars of a bicycle or motorcycle, with 20 cases reported to date in the English language literature, 12 in children. We report two new cases of handlebar hernia, in children aged 6 and 10 years. In both cases, physical examination revealed an area of contusion and bruising in the lower abdomen. However, the muscle defect was detected during the first examination in only one of the patients, and not until several days later in the other patient. Abdominal ultrasonography proved useful for diagnosis in both patients. Early surgical correction is necessary to prevent possible complications. This type of hernia should be borne in mind when evaluating children who have suffered abdominal trauma in a bicycle accident.
Résumé
Les hernies traumatiques de l'abdomen sont exceptionnelles chez l'enfant. La plupart résultent d'un traumatisme direct sur un guidon de bicyclette ou de motocyclette. Des vingt cas décrits dans la littérature, 12 sont des cas pédiatriques. Nous présentons deux nouveaux cas de traumatismes directs sur guidon de bicyclette chez des enfants âgés de 6 à 10 ans. L'examen clinique mettait en évidence une aire de contusion et d'ecchymose abdominale dans la région inférieure de l'abdomen. Cependant, un défaut de musculature n'était pas retrouvé à la première exploration sauf chez l'un des enfants. Il n'était détectable que quelques jours plus tard chez le deuxième. L'ultrasonographie abdominale s'est révélée utile pour le diagnostic dans les deux cas.
La correction chirurgicale précoce est nécessaire pour prévenir de possibles complications. Ce type d'hernie est à envisager devant tout enfant ayant des antécédents de traumatisme abdominal dû à un accident de bicyclette.
Resumen
Las hernias abdominales traumáticas localizadas son raras. La mayoría se deben a un impacto directo contra el manillar de una bicicleta o motocicleta. Hay al menos 20 casos descritos en la literatura inglesa de los que 12 correspondían a niños. Presentamos dos nuevos casos de hernia del manillar en niños de 6 y 10 años. En ambos casos la exploración física reveló un área de contusión y equimosis abdominal en la región inferior del abdomen. Sin embargo, el defecto muscular sólo se encontró en la primera exploración en uno de ellos no detectándose en el otro hasta varios días después. La ecografía abdominal resultó útil para el diagnóstico en los dos casos. Es necesaria la corrección quirúrgica temprana para evitar posibles complicaciones. Este tipo de hernia debe ser tenido en cuenta ante todo niño con antecedente de traumatismo abdominal por accidente de bicicleta.

Lokalisierte, abdominelle Traumen sind selten. Sie werden meist verursacht durch einen Fahrrad- oder Motorradlenker, wobei in der englischsprachigen Literatur 20 Fälle bei 12 Kindern berichtet wurden.
In der vorliegenden Arbeit wird über Bauchwandverletzungen bei 2 Kindern im Alter von 6 und 10 Jahren berichtet. In beiden Fällen kam es zu einer starken Kontusion und Ruptur der Bauchwand im unteren Abdominalbereich. Jedoch nur bei einem der Kinder wurde ein Bauchwanddefekt bei der primären Untersuchung entdeckt, bei dem anderen einige Tage später. Die Ultraschalluntersuchung hat sich als nützliches diagnostisches Hilfsmittel in beiden Fällen erwiesen. Eine frühe chirurgische Korrektur ist notwendig, um Komplikationen bei dieser seltenen Hernienform zu verhindern.
