Soziale Ungleichheit im Zugang zur ambulanten Behandlung in England und Deutschland

Zusammenfassung

Es gibt deutliche soziale Unterschiede in Krankheit und Sterblichkeit in England und in Deutschland. Ein Ansatz zur Verminderung besteht in der einfachen und ungehinderten Zugänglichkeit zur medizinischen Versorgung. Untersucht wurde an dem Englischen und dem Deutschen Haushalts-Survey (GHS und BGS), ob es in der Eingangsforte zum System, bei dem ambulanten Arzt/Hausarzt, soziale Unterschiede im Zugang in diesen beiden Länder mit unterschiedlichen Medizinkulturen und unterschiedlicher Aufmerksamkeit der sozialen Frage gegenüber gibt. Untersucht wurde dies in Bezug auf den Bedarf. Hierfür wurden nur die Personen analysiert, die eine chronische, einschränkende Erkrankung im Survey angegeben haben. Befund ist: Es gibt auf dieser Versorgungsebene in beiden Ländern keine sozialen Unterschiede in der Nutzung - dies auch nicht, wenn man die Zeitverläufe von 1984 bis Mitte der 90iger Jahre betrachtet.

Abstract

There is relevant social inequality in morbidity and mortality in Germany and in Britain. An approach to reduce these inequalities could be to make the health care sector equally accessible for all social groups. The study uses the British and the German Household Survey (GHS and BHS) to analyze social equality in visiting a GP or - in Germany - a GP or another non-hospital doctor. For this purpose, attendance of a doctor by patients with “chronic and limiting” illnesses are analysed (use/need ratio). Germany and Britain differ in their medical culture and their different emphasis on the “social problems” within their respective societies. The study's finding is that in both countries, despite the different approaches, there are no social differences in the use/need ratio of GP-services/out of hospital care. This also was true when analyzing the time trend from 1984 to the mid 1990s.
