Duplication Cyst of the Stomach Presenting as Hemoptysis

Abstract

We report a rare case of a 2-year-old boy with a gastric duplication cyst located in the stomach, presenting with vague abdominal pain, recurrent cough, hemoptysis and a lesion in the left lower lobe of the lung reported as a sequestration. The duplication cyst was attached to the diaphragm and lungs by a narrow tract. The cyst was excised. Subsequent scans of the chest showed regression of the lung lesion. We believe this to be the first case in the literature where the hemoptysis was cured and the pulmonary lesion regressed completely after excision of the duplication cyst of the stomach alone. Pulmonary resection was thus avoided.

Résumé

Nous rapportons le cas rare d'un garçon de 2 ans avec une duplication gastrique localisée à l'estomac. Il s'est présenté avec des douleurs abdominales, une toux, une hémoptysie et une lésion du lobe inférieur gauche du poumon considéré comme une séquestration. La duplication kystique était attachée au diaphragme et au poumon par un mince tractus. Le kyste a été excisé. Des scanners successifs ont montré la régression de la lésion pulmonaire. Nous pensons que ceci est le premier cas dans la littérature, de la guérison d'une hémoptysie et d'une lésion pulmonaire après excision complète d'une duplication kystique solée de l'estomac seulement. La résection pulmonaire a pu être évitée.

Resumen

Presentamos un caso raro de un niño de dos años con una duplicación quística localizada en el estómago que comenzó con dolor abdominal vago, tos recurrente, hemoptisis y una lesión en el lóbulo inferior izquierdo del pulmón que se diagnosticó como secuestro. La duplicación quística estaba adherida al diafragma y a los pulmones por un tracto estrecho. Se escindió el quiste. El scanner posterior mostró una regresión de la lesión quística. Creemos que éste es el primer caso de la literatura en el que se curó la hemoptisis y la lesión pulmonar desapareció completamente tras la eliminación de la duplicación quística del estómago. Se evitó así la resección pulmonar.

Zusammenfassung

Ein zweijähriger Knabe mit einer Magenduplikatur wird vorgestellt. Die Erkrankung äußerte sich in rezidivierenden Bauchschmerzen, Husten und Bluterbrechen. Gleichzeitig wurde eine Lungensequestration im linken Lungenunterlappen diagnostiziert. Die Duplikatur war mit dem rechten Zwerchfell adhärent und mit dem Sequester durch einen engen Kanal verbunden. Bei der Laparatomie wurde die Zyste exzidiert, die Lungenläsion jedoch belassen. In der Folge schrumpfte der Lungenprozess vollständig.

Schlussfolgerung: Dies ist der erste Fall in der Literatur, bei dem Symptome einer Magenduplikatur durch Exzision vollständig geheilt werden konnten und die gleichzeitig bestehende pulmonale Sequestrierung ebenfalls spontan regredient ist, so dass eine Resektion vermeidbar wurde.
