Zusammenfassung

Durch ein dichtgeknüpftes palliatives Versorgungsnetz, das möglichst alle Bedürfnisse des Patienten am Lebensende erfasst und diese auch therapeutisch erfüllt, ist nach den Erkenntnissen aus bisher eingeführten ambulanten Versorgungskonzepten eine ambulante Versorgung des Patienten bis zuletzt zu einem hohen Prozentsatz zu Hause möglich. Während in Deutschland zur Zeit nur 20-25 % der Patienten zu Hause versterben, weil gerade diese Infrastruktur fehlt, belegen die Zahlen aus gut funktionierenden ambulanten Palliativdiensten, dass bis zu 70 % der Patienten wunschgemäß zu Hause versterben können. Da in Deutschland gerade im Bereich der häuslichen Versorgung unstrittig die größten Defizite liegen, besteht gerade hier ein besonderer medizinischer wie gesellschaftlicher Handlungsbedarf.

Summary

As concepts for ambulatory patient care have shown, with the aid of a close-knit palliative care network taking account of, and also meeting, as many of the terminal patient's therapeutic needs as possible, many patients can be offered domiciliary care right up to the end. While, since the infrastructure described is not in place, only 20-25 % of patients in Germany currently, die at home, figures issued by well-functioning ambulatory palliative care services show that, in conformity with their wishes, up to 70 % of the patients die at home. Since, in the field of palliative medical care in Germany, the largest deficits are undoubtedly to be found in domiciliary care, there is a particular need to be met, both medical and societal.
