Zusammenfassung
Rückenschmerzen werden oft mit der Funktionsstörung Hypermobilität/Instabilität assoziiert.
Eine erfolgreiche Behandlung muss nach etablierter Diagnose einem prinzipiell rigiden,
vorgegebenen Schema folgen. Es hat sich die Erkenntnis durchgesetzt, dass nach erfolgreichem
Beheben der Dysfunktion des lokalen Muskelsystems zwingend ein Training der globalen
Muskulatur zur Kompensation der Dekonditionierung sowie des Verlusts der globalen
Stabilität unter Berücksichtigung der Patientenbedürfnisse notwendig ist. Zudem bestätigen
Studien zur Evidenz konservativer Therapie bei chronischem Rückenschmerz die Wirkung
von Trainingsmaßnahmen. Dieser Artikel beschreibt den Trainingsaufbau bei schmerzhaften
Funktionsstörungen der LWS nach erfolgreichem Training zur Verbesserung der lokalen Stabilität. Die globale Stabilisation
beinhaltet 5 Phasen. Der größte Teil des Trainingsaufbaus basiert auf dem Trainingsgerät
Total Gym, dessen Einsatz vorgestellt wird.
Abstract
Low back pain is often associated with dysfunction of hypermobility/instability. Following
the establishment of the diagnosis, successful treatment has to conform to a defined,
in principle rigid, specified programme. It has been realised that after successful
elimination of the local muscle system's dysfunction, global muscular training under
consideration of the patient's requirements is absolutely necessary in order to compensate
for deconditioning as well as for the loss of global stability. In addition, studies
examining the evidence for conservative therapy for chronic low back pain have shown
the efficacy of therapeutic exercise. This article describes an exercise programme
for painful lumbar spinal dysfunction after successful therapeutic exercise has improved
local stability. Global stabilisation comprises 5 phases. The main part of the exercise
programme is determined by the use of the exercise apparatus total gym. The application of the total gym is described.
