Zusammenfassung
Die minimalinvasiven Operationsverfahren nehmen einen immer höheren Stellenwert in der Endoprothetik ein. Die hier vorgelegte Arbeit beschäftigt sich mit der Frage, ob auch bei rheumatoider Arthritis die Knieendoprothese minimalinvasiv eingebracht werden darf. Hierfür wurden insgesamt 75 Patienten retrospektiv im Rahmen einer Pilotuntersuchung ausgewertet. Insbesondere wurde das früh funktionelle Ergebnis analysiert, die Patienten wurden im Abstand von 7 Tagen postoperativ nach einem hauseigenen Score untersucht. Dabei zeigte sich, dass die minimalinvasiv operierten Patienten sowohl in der Arthrose- als auch in der Arthritisgruppe eine deutlich raschere Rehabilitation aufwiesen. Sie erreichten schon nach 4 Tagen Rehabilitationswerte, die üblicherweise erst nach zwei Wochen erreicht werden, deshalb könnte die stationäre Behandlung verkürzt werden. Andererseits treten Komplikationen auf bei Rheumatikern, insbesondere im Bereich der Haut. Die Implantationsgenauigkeit ist gegenüber den Standardverfahren verringert. Flexionen der Femurkomponente und Lateralisation des Tibiaplateaus wurden in Einzelfällen beobachtet. Deshalb ist die Anfertigung prospektiv randomisierter extern evaluierter Studien erforderlich, um eine abschließende Aussage zur Fragestellung zu ermöglichen. Rheumakranke Patienten können minimalinvasiv operiert werden, die frühfunktionelle Analyse zeigt gegenüber der Arthrosegruppe nicht signifikant bessere Ergebnisse.
Abstract
Minimally invasive surgical procedures are gaining increasing importance in endoprosthetics. The present article addresses the question of whether knee joint endoprostheses may be minimally invasively implanted in cases of rheumatoid arthritis. In the framework of a pilot study the records of 75 patients were evaluated retrospectively. In particular, the early functional results were analysed; the patients were examined at 7-day intervals postoperatively using a score designed in our institution. It was found that the minimally invasive operated patients in both the arthrosis group and the arthritis group experienced a markedly earlier rehabilitation. After as little as 4 days they achieved rehabilitation values that are usually only reached after 2 weeks, thus the indoor treatment could be shortened. On the other hand, complications, especially of the skin, arose among the rheumatic patients. The accuracy of implantation is lower in comparison to standard procedures. In individual cases, flexions of the femur components and lateralisation of the tibial plateau were observed. Thus, the performance of prospective, randomised, externally evaluated studies is necessary in order to obtain conclusive answers to the posed question. Pateints with rheumatic diseases can undergo minimally invasive operations of the knee, however, the early functional results are not significantly better than those of the arthrosis group.
