Abstract
Hepatic mesenchymal hamartoma is a rare benign tumour in children. It is often large and centrally located in the liver at diagnosis, making surgical resection difficult; thus non-radical resection has been proposed in the past as acceptable management. However, a literature survey and a case with recurrence associated with cytogenetic anomalies suggest that radical liver surgery (resection with a margin of normal liver parenchyma, as for malignant tumour) should be recommended for mesenchymal hamartoma.
Résumé
L'hamartome mésenchymateux du foie est une tumeur bénigne rare de l'enfant. La résection peut être difficile en raison de sa taille et de sa position habituellement centrale dans le foie, et leur traitement par résection subtotale a été proposé de par le passé. Néanmoins, la présence d'anomalies cytogénétiques dans certains cas, et la possibilité de récurrence ou de transformation tumorale, doit encourager le chirurgien à considérer une chirurgie radicale dans tous les cas.
Resumen
El hamartoma mesenquimal hepático es un tumor benigno raro en niños. A menudo es grande y central lo que hace difícil la resección quirúrgica. Por este motivo se han propuesto en el pasado resecciones no radicales como opciones aceptables. Sin embargo, una revisión de la literatura y un caso de recidiva asociado con anomalías citogenéticas sugiere que el tratamiento recomendable es la extirpación radical con un margen de parénquima libre como en los tumores mailgnos.
Zusammenfassung
Ein mesenchymales Hamartom ist ein seltener Tumor bei Kindern. Bei Diagnosestellung ist es oft bereits sehr groß und zentral in der Leber lokalisiert, was die chirurgische Resektion erschwert; so wurde bisher die nichtradikale Resektion als geeignete Therapie angesehen. Eine Literaturübersicht und ein Fall mit Wiederauftreten des Tumors, verbunden mit zytogenetischen Anomalien, bringt uns zu dem Schluss, dass radikale Leberchirurgie (Resektion im gesunden Lebergewebe, wie für maligne Tumore) bei mesenchymalen Hamartomen vorzuziehen ist.
