Intra-Articular Recurrence of Benign Chondroblastoma with Articular Involvement at Initial Presentation - A Case Report

Abstract

We report a case of chondroblastoma involving the upper tibial epiphysis, which had an unusually aggressive course, with articular involvement at initial presentation. Intra-articular and soft tissue recurrence occurred after treatment consisting of curettage and bone grafting. Although the incidence of local intraosseous recurrence of chondroblastoma is relatively high, intra-articular and soft tissue implantation is rare and is usually due to intra-articular spillage during surgery. Spontaneous articular involvement is rarely seen.

Résumé

Nous rapportons un cas d'un chondroblastome touchant l'épiphyse tibiale supérieure qui a eu une évolution inhabituellement agressive avec altération articulaire initiale. La récidive intra-articulaire et des tissus mous survint après le traitement, consistant en un curetage et une greffe osseuse. Quoique l'incidence de la récidive locale intra-osseuse du chondroblastome est relativement haute, la récidive intra-articulaire et des tissus mous est rare et habituellement due à des disséminations intra-articulaires durant la chirurgie. L'altération articulaire spontanée est rare.

Resumen

Presentación de un caso de condroblastoma que afectaba a la epífisis tibial superior con afectación articular desde el inicio y con un curso clínico extraordinariamente agresivo. La recidiva intraarticular y del tejido blando ocurrió tras tratamiento consistente en curetaje e injerto óseo. Aunque la incidencia de recidiva intraósea del condroblastoma es relativamente alta, la recidiva intraarticular y en tejido blando es rara y se debe generalmente a diseminación intraarticular durante la cirugía. La afectación articular espontánea es muy rara.

Zusammenfassung

Fallbeschreibung eines aggressiven Chondroblastoms der oberen Tibiaepiphyse mit primärer Gelenkbeteiligung. Nach therapeutischer Curettage und Knochentransplantation traten sowohl ein Weichteil- als auch ein Gelenkrezidiv auf. Im Gegensatz zu intraossären Rezidiven mit einer eher relativ hohen Inzidenz von 5 - 38 % treten Gelenk- und Weichteilrezidive selten auf und sind gewöhnlich auf eine intraoperative Aussaat zurückzuführen. Eine primäre Gelenkbeteiligung wird selten beobachtet.
