Zusammenfassung
Der frühe Behandlungsbeginn mit Basistherapeutika hat in den letzten Jahren einen besonders großen Effekt auf den Krankheitsverlauf gezeigt. Mit dieser frühen Therapieplanung bleibt man im „window of opportunity”, das nur zu Krankheitsbeginn geöffnet ist. Neben dem frühen Beginn spielt auch die Intensität der Medikation eine Rolle: bei nicht ausreichender Wirksamkeit wird das Präparat gewechselt beziehungsweise eine Kombination von zwei oder drei Substanzen gewählt. Die neun gebräuchlichsten konventionellen Basistherapeutika sind Methotrexat (MTX), Sulfasalazin, die Antimalariamittel Chloroquin und Hydroxychloroquin, Leflunomid, Parenterales Gold, Cyclosporin A, Azathioprin, Cyclophosphamid und D-Penicillamin. Ein wesentliches Charakteristikum der Basistherapeutika ist der verzögerte Wirkungseintritt nach zirka 4-6 Wochen beziehungsweise nach 4-6 Monaten. Für die Verlaufsbeurteilung sind neben den klinischen und laborchemischen Untersuchungen auch Röntgenbilder der betroffenen Gelenkregionen im Abstand von wenigstens sechs, im Mittel 24 Monaten angezeigt.
Summary
In recent years, the early initiation of treatment with DMARDS has demonstrated an impressive positive impact on the course of the disease. With such early therapy planning, the physician takes advantage of the „window of opportunity”, which is open only in the early stages of the disease. Apart from early initiation, the intensity of medication therapy also plays a role: in the event of an inadequate effect, the substance is changed or a combination of two or three substances applied. The nine most common conventional DMARDs are methotrexate (MTX), sulphasalazine, the antimalarials chloroquine and hydroxychloroquine, leflunomide, parenteral gold, cyclosporine A, azathioprine, cyclophosphamide and D-penicillamine. A leading characteristic of the DMARDs is a delayed onset of effect after approximately 4-6 weeks or after 4-6 months. In order to assess the course of the disease, not only clinical and laboratory tests, but also X-rays of the affected joints obtained at intervals of at least 6 - on average, 24 - months are indicated.
