Nicht-Steroidale-Anti-Rheumatika (NSAR) bei entzündlich rheumatischen Erkrankungen

Zusammenfassung

Die NSAR sind essentielle Medikamente in der kurz- und mittelfristigen Behandlung von entzündlich rheumatischen Erkrankungen. Auf ihre entzündungshemmende Wirkung, die Reduktion der Prostaglandinsynthese, kann nicht verzichtet werden. Mit entsprechender Dosisanpassung sind die NSAR vom Säuglingsalter an bis zum Senium einsetzbar. Bei einer länger dauernden Therapie sind die speziellen Risiken dieser Medikamente wie Magen-Darmschädigung und die kardiovaskulären Komplikationen besonders zu beachten. Die Nebenwirkungen lassen sich aber durch eine geschickte Auswahl der Präparate (bezüglich Selektivität, Galenik und Dosis) sowie durch eine Magenschutzprophylaxe bei Risikogruppen deutlich reduzieren.

NSAIDS continue to be essential drugs in the short-term and medium-treatment of inflammatory rheumatic diseases, since their inflammation-inhibiting effect via the reduction of prostaglandin synthesis is indispensable. Provided that the dose is appropriately selected, NSAIDs can be employed for patients ranging from infants to old people. In the case of long-term treatment, the particular risks of the drugs, such as gastrointestinal damage and cardiovascular complications needs to be carefully considered. However, by appropriately choosing the substances (with regard to their selectivity, galenic properties and dose) and the use of gastric protective measures in high-risk groups, side effects can be appreciably reduced.
