Abstract
Purpose: The aim of this study is to report on the late results of the modified surgical procedure “subtotal splenectomy with omentopexy and partial splenic attachment to the parietal peritoneum” for the treatment of portal hypertension in children. Methods: Over a period of twenty years (1984 - 2004), two modifications have been performed of the procedure of “partial splenectomy and partial splenic attachment for the treatment of portal hypertension”, which were reported in the Zeitschrift für Kinderchirurgie [[9]], in 1990. The modifications were due to the recurrence of splenomegaly in five cases, with signs of hypersplenism in three of them after ten years. Results: The follow-up ranges between one and fourteen years. All the patients are in good health, with good growth and development, except for one case with liver failure due to liver cirrhosis. No mortality occurred in this group of patients. Severe bleeding occurred in only one case after four years. Treatment consisted of sclerotherapy and ligation of the splenic artery. Splenomegaly was found in five cases, with hypersplenism in three of them. Splenic artery embolisation in two cases and ligation in one case produced good results. Howell-Jolly bodies were negative in all cases and radionuclide scan showed satisfactory to good results in most cases. Good results were also obtained in the studies of immunoglobulins and complements in most cases, with two exceptions of reduced IGM in one case and C4 in the other. Conclusions: Subtotal splenectomy with omentopexy and partial splenic attachment is a good alternative to other operative procedures for the treatment of portal hypertension. With this method, the precipitation and increase in collateral circulation leads to rapid improvement.
Résultats à long terme d'un nouveau procédé chirurgical pour le traitement de l'hypertension portale
Buts: Le but de cette étude est de reporter les résultats de procédés chirurgicaux: «splénectomie subtotale avec fixation du grand épiplon et fixation partielle de la rate au péritoine pariétal» qui a été utilisée pour le traitement de l'hypertension portale chez 16 enfants. Méthodes: Sur une période de 20 ans (1984 - 2004), deux modifications ont été apportées à cette technique qui ont été rapportées dans le Zeitschrift für Kinderchirurgie en 1990. Les modifications étaient dues à une récidive de la splénomégalie dans 5 cas avec des signes d'hypersplénisme dans 3 cas après une dizaine d'années. Résultats: Le suivi se situe entre 1 et 14 ans. Tous les patients sont en bonne santé avec un bon développement, excepté pour un cas d'insuffisance hépatique avec cirrhose. Il n'y a pas de mortalité. Une hémorragie sévère est survenue dans seulement un cas après 4 ans. Le traitement a consisté en une sclérothérapie et ligature de l'artère splénique. Une splénomégalie a été retrouvée dans 5 cas avec hypersplénisme dans 3 cas. L'embolisation de l'artère splénique dans deux cas et la ligature de celle-ci dans un cas a produit de bons résultats. La scintigraphie a montré des résultats satisfaisants dans la plupart des cas. De bons résultats étaient aussi obtenus en étudiant les immunoglobulines et le complément de la plupart des cas avec deux exceptions où l'on retrouvait une réduction des immunoglobulines dans un cas et du C4 dans l'autre. Conclusion: La splénectomie subtotale avec fixation du grand épiplon et attachement au péritoine pariétale est une bonne alternative aux autres traitements chirurgicaux de l'hypertension portale. Avec cette méthode, l'augmentation de la circulation collatérale conduit à une amélioration rapide.
Resultados tardíos de un procedimiento nuevo modificado para el tratamiento de la hipertensión portal
Objetivos: Presentar los resultados tardíos de una técnica quirúrgica modificada (esplenectomía subtotal con omentopexia y esplenopexia parcial a la pared) para el tratamiento de la hipertensión portal en 16 niños. Métodos: Entre 1984 y 2004 se han introducido 2 modificaciones de esta técnica inicialmente publicada en el Zeitschrift für Kinderchirurgie en 1990. Las modificaciones se realizaron a causa de la recidiva de la esplenomegalia en 5 casos con signos de hiperesplenismo en 3 de ellos 10 años después. Resultados: El seguimiento fue de entre 1 y 14 años. Todos los pacientes están en buena salud con buen crecimiento y desarrollo excepto en uno que tenía fallo hepático por cirrosis. No hubo mortalidad. Ocurrió hemorragia grave solamente en un caso en 4 años. El tratamiento consistió en escleroterapia y ligadura de la arteria esplénica. Se encontró esplenomegalia en 5 casos con hiperesplenismo en 3 de ellos. Se hizo embolización de la arteria esplénica en 2 casos y ligadura en 1 lo que produjo buenos resultados. Los cuerpos de Howell -Jolly fueron negativos en todos los casos y la escintigrafia mostró resultados satisfactorios a buenos en la mayoría de los casos. También se obtuvieron buenos resultados en estudios de inmunoglobulinas y complemento en la mayoría de los casos con 2 excepciones en las que había reducción de IgM y de C4. Conclusiones: La esplenectomía subtotal con omentopxia y esplenopexia a la pared es una buena alternativa a otros procedimientos operatorios para el tratamiento de la hipertensión portal. Con este método el aumento de la circulación colateral lleva a una rápida mejoría.
Spätergebnisse einer modifizierten neuen Operationstechnik zur Behandlung der portalen Hypertension
Zielsetzung: Ziel der Studie war es, die Spätergebnisse nach dem neuen operativen Verfahren einer subtotalen Splenektomie mit Omentopexie und partieller Anheftung der Milz am parietalen Peritoneum bei Kindern mit portaler Hypertension zu schildern. Methodik: In einem Zeitraum von 20 Jahren (1984 - 2004) wurden zwei Modifikationen der partialen Splenektomie mit Milzfixation am Peritoneum bei portaler Hypertension entsprechend einer Publikation in der Zeitschrift für Kinderchirurgie (1990) durchgeführt. Die Modifikation war notwendig, da es zu einem Rezidiv der Splenomegalie bei fünf Patienten mit Zeichen eines Hypersplenismus bei drei von ihnen nach zehn Jahren gekommen war. Ergebnisse: Die jetzt berichteten Nachuntersuchungsergebnisse betreffen einen Zeitraum von 1 - 14 Jahren nach dem operativen Eingriff. Alle Patienten befinden sich in guter Gesundheit, mit normalem Wachstum und Entwicklung mit Ausnahme eines Kindes mit Leberversagen aufgrund einer Leberzirrhose. Es gab keine Mortalität. Schwere Blutungen traten nur in einem Fall nach vier Jahren auf. Die Behandlung bestand in einer Sklerotherapie und Ligatur der Milzarterie. Eine Splenomegalie wurde bei 5 Fällen beobachtet, bei dreien mit Hypersplenie. In zwei Fällen konnte eine arterielle Embolisation und in einem Fall eine Ligatur der Milzarterie dennoch zu guten Ergebnissen führen. Howell-Jolly-Körperchen waren bei allen Kindern negativ. Die Milzszintigraphie ergab befriedigende bis gute Ergebnisse bei allen Fällen. Auch im Hinblick auf die Immunglobuline und Komplementfaktoren zeigten alle Patienten bis auf zwei mit verringerten IGM-Spiegeln und ein Kind mit verrringertem C4-Spiegel Normalbefunde. Schlussfolgerungen: Eine subtotale Splenektomie mit Omentopexie und partieller Milzanklebung am parietalen Peritoneum ist eine gute Alternative für andere Operationsmethoden zur Behandlung der portalen Hypertension. Diese Methode beschleunigt die Entwicklung eines Kollateralkreislaufes und führt somit zu einer schnellen Verbesserung des portalen Hochdrucks.
