Zusammenfassung
Wenn auch auf den ersten Blick ungewöhnlich, macht es durchaus Sinn, Sinusthrombosen und durale AV-Fisteln in einer gemeinsamen Arbeit abzuhandeln. Zum einen sind beide Krankheiten klinisch ein bisschen ein Chamäleon und können zu einer Vielzahl von Symptomen führen. Haben sie einmal eine intrazerebrale Blutung verursacht - und das kann bei beiden Krankheiten passieren - wird meistens eine Angiographie die Klärung bringen. Durale AV-Fisteln entstehen nach heutigen Vorstellungen überwiegend als Folge eines erhöhten Venendrucks. Ursache kann - in seltenen Fällen - ein venöses Abflusshindernis in Form einer venösen Stenose sein. Häufiger wird eine abgelaufene Sinusthrombose mit überschießender Rekanalisierung und Ausbildung von Mikroshunts zwischen den Durablättern die Ursache der duralen AV-Fistel sein. Unterschiedlich ist die radiologische Diagnostik: die Schnittbilddiagnostik ist die Domäne bei der Sinusvenenthrombose, die Angiographie hier eher die Ausnahme. Bei den duralen AV-Malformationen ist es genau umgekehrt. Auch die therapeutische Strategie ist völlig unterschiedlich: die intravenöse Heparinisierung ist fast immer der therapeutische Ansatz bei Sinusvenenthrombosen, interventionelle Therapietechniken sind die absolute Ausnahme. Bei duralen AV-Malformationen, insbesondere solchen mit einem Blutungsrisiko, steht die endovaskuläre Therapie hingegen ganz im Vordergrund.
Abstract
Even if it is unusual at a first look, it does make sense to review sinovenous thrombosis and dural AV fistulas together. Both diseases are a chameleon on one side and can lead to many different symptoms. If they cause an intracerebral bleeding - that can occur in both - diagnostic angiography is necessary. From a recent point of view dural AV fistulas are the consequence of an increased venous pressure. Rarely, they are caused by a venous obstruction or a venous stenosis. More often a sinovenous thrombosis with consecutive overwhelming recanalisation and formation of microshunts between the dural layers will be the cause of a dural fistula. The diagnostic work-up is totally different: cross sectional imaging is first line in sinovenous thrombosis and angiography the exception. The opposite is true with dural AV fistulas. The therapeutic approach is also totally different in both diseases: intravenous heparinisation is almost always the therapy of choice in sinusvenous thrombosis and interventional techniques are the exception. In dural AV fistulas, specifically in those with an increased bleeding risk, endovascular therapy is first choice.
