Zusammenfassung
Weltweit stellen nosokomiale Infektionen eines der größten Probleme stationärer Einrichtungen dar. Der Anteil der Methizillin-resistenten Staphylococcus-aureus-Isolate (MRSA) stieg in den letzten Jahren auch in Deutschland kontinuierlich an. Verschiedene Studien benennen in Deutschland in 2005 einen Anteil von 20 bis 35 % MRSA am Gesamtanteil aller nachgewiesenen Staph.-aureus-Erreger. Eine effektive Infektionskontrolle des MRSA, aber auch anderer Keime mit Resistenzentwicklungen, ist nur durch gemeinsame Präventionsstrategien der drei Säulen des Gesundheitswesens, stationärer Behandlung, ambulanter Behandlung und öffentlicher Gesundheitsdienst zu erreichen. MRSA-Infektionen wie Pneumonie, Sepsis und Wundinfektionen führen zu verlängerten Krankenhausaufenthalten, erhöhter Morbidität, höheren Kosten und auch zum Versterben des Patienten. Einigen europäischen Ländern ist es gelungen, durch konsequente Präventionsmaßnahmen die Ausbreitung von MRSA und MRSA-Infektionen drastisch zu reduzieren. Eine Reduktion könnte auch bei uns noch gelingen, da sowohl die Quelle als auch der Übertragungsweg bekannt ist. Meist handelt es sich um infizierte oder besiedelte Patienten mit typischem Risikoprofil. Fehlendes Aufnahmescreening von Risikopatienten im Krankenhaus, mangelhafte Dokumentation bekannter MRSA-Träger und ein unvollständiges Entlassungsmanagement im Umgang mit der Weitergabe von Informationen zur MRSA-Besiedelung sind wichtige Gründe, warum es immer wieder unerkannt zu Transmissionen kommt. Hier kann im Rahmen der hausärztlichen Versorgung bei der Einweisung, nach Krankenhausentlassung, in Alten und Pflegeheimen und beim Einsatz von Antibiotika wirksamer Einfluss auf die weitere Ausbreitung multiresistenter Erreger genommen werden.
Abstract
Throughout the world nosocomial infections turn out to be one of the most severe problems in hospital treatment. Germany had to face a continuously rising percentage of Methicillin-resistent Staphylococcus aureus isolates (MRSA) during the last years. Several studies reveal a percentage of 20 up to 35 percent MRSA of all proven Staphylococcus aureus. An efficient control of MRSA infections - including those of other germs that may develop resistances - can only be achieved through cooperation of primary and secondary care as well as community medicine. MRSA infections such as pneumonia, sepsis and infected wounds enhance hospitalisations, morbidity, costs and last but not least mortality. In some European countries however it proved to be possible to drastically reduce MRSA by consequent means of prevention. Such a reduction could be possible in Germany too, since the source as well as the path of contamination is well known. In most cases affected patients share a typical profile of risks. Transmissions often remain unrealized due to a lacking screening of risk patients at the time of hospital attendance, a poor documentation of known MRSA carriers and an ineffective communication on MRSA carriers at the time of dismissal from hospital. General practitioners can have an impact on the widening spread of germs by better communicating with hospital during admission and after discharge as well as in nursing homes and by a wise and careful use of antibiotics.
