Zusammenfassung
Anliegen Die transkranielle Magnetstimulation erlaubt die nichtinvasive Untersuchung der kortikalen Exzitabilität und dabei insbesondere die differenzierte Erfassung inhibitorischer Mechanismen. Methode Verschiedene Parameter der kortikalen Erregbarkeit (motorische Schwelle, intrakortikale Inhibition und Fazilitation, Cortical Silent Period) wurden bei 10 unbehandelten Patienten mit schwerer Depression bestimmt und mit den Ergebnissen einer hinsichtlich Geschlechts- und Altersverteilung kontrollierten Kontrollgruppe verglichen. Ergebnisse Bei den depressiven Patienten fand sich im Vergleich zur Kontrollgruppe eine signifikant erhöhte intrakortikale Inhibition. Andere Parameter der kortikalen Erregbarkeit unterschieden sich nicht. Schlussfolgerungen Diese Befunde stützen die Hypothese, dass depressive Erkrankungen mit Funktionsveränderungen zentral inhibitorischer Mechanismen assoziiert sind.
Abstract
Objective Transcranial magnetic stimulation (TMS) provides a non-invasive tool for the investigation of motor cortex excitability. Especially this method holds potential for the differential assessment of inhibitory and facilitatory cortical circuits. Methods Several parameters of cortical excitability (Motor threshold, intracortical inhibition and facilitation, cortical silent period) were assessed in 10 patients with severe major depression (MDD; DSM-IV) off medication. Results were compared with data from healthy controls matched for age and sex. Results In the group of severly depressed patients intracortical inhibition was significantly increased compared to healthy controls, whereas other TMS paradigms failed to show significant differences between patients and controls. Conclusions Our findings support the notion that major depressive disorder is associated with alterations of central inhibitory mechanisms.
