Compliance - Zur Funktion eines Begriffes der medizinischen Alltagspraxis

Zusammenfassung

Ausgehend von einer Rekapitulation der Ergebnisse der Complianceforschung wird der Frage nachgegangen, welche Funktion der Compliancebegriff in der medizinische Routinepraxis erfüllt, in der Compliance meist reduziert als Befolgung ärztlicher Anordnungen im Rahmen eines paternalistischen Arzt-Patient-Verhältnisses verstanden wird. In psychotherapeutischer und sozialmedizinischer Perspektive wird deutlich, dass ein solches reduziertes Complianceverständnis der Verdrängung psychischer und sozialer Konflikte in der therapeutischen Beziehung und einem „Compliancegewinn” auf beiden Seiten dieser Beziehung dient.

Abstract

Based on findings of research on compliance the function of the concept of compliance in medical practice is discussed. In medical practice compliance is understood as obedience of patients to doctor's order in the framework of a paternalistic doctor-patient-relationship. In a psychotherapeutic view and a perspective of social medicine this concept appears to be a reductionist concept which serves the repression of psychic and social conflicts in the therapeutic relationship. At the same thime it leads to a ”gain from compliance” on both sides of this relationship.
