Geschlechtsspezifische Aspekte in der Pharmakotherapie - was ist gesichert?

Zusammenfassung

Geschlechtsspezifische Unterschiede in der Arzneimitteltherapie können in vier Kategorien eingeteilt werden. 1. Es kann das Ansprechen auf eine Therapie zwischen Männern und Frauen unterschiedlich ausgeprägt sein, weil der Arzneistoff unterschiedlich metabolisiert wird und dementsprechend die Wirkung oder Wirkdauer modifiziert wird. 2. Die Empfindlichkeit der Zielstrukturen wie Rezeptoren oder Kanäle ist geschlechtsspezifisch unterschiedlich, Beispiele hierfür sind die kardiale Reizleitung mit gehäufter QT-Verlängerung bei Frauen. 3. Die Pathophysiologie der Erkrankung als solche ist geschlechtsspezifisch, dies spielt z. B. im Herz/Kreislaufsystem und bei psychiatrischen Erkrankungen eine Rolle. 4. Auf einer soziomedizinischen Ebene sind Unterschiede in der Wahrnehmung und Beschreibung von Symptomen und im Umgang mit Krankheiten zu berücksichtigen. Häufig ist es schwierig, biologische und soziomedizinische Aspekte zu differenzieren, weil Untersuchungen zu Grundlagen der geschlechtsspezifischen Unterschiede in vielen Bereichen fehlen. Ein zusätzlicher Fallstrick ergibt sich durch die Tatsache, dass geschlechterspezifische Unterschiede bei Erkrankungen und Arzneimitteln auftreten, bei denen man primär nicht an hormonelle Einflüsse denkt - wie z. B. dem Myokardinfarkt. Dennoch können für den praktisch tätigen Arzt zumindest Hinweise gegeben werden, in welchen Bereichen eine geschlechtersensible Verordnung oder Überwachung einer Therapie in Erwägung gezogen werden sollte.

Abstract

Gender- or sex-specific differences in pharmacotherapy can be divided in four major categories. 1. Treatment response may differ between men and women due to differences in drug metabolism resulting in modifications of the intensity or the duration of the effect. 2. Sensitivity of target structures of drugs such as receptors or channels may differ between sexes, e. g. QT-prolongation as an adverse effect occurs more frequently in women then in men. 3. The pathophysiology of diseases may be sex-specific. This plays a role e. g. in cardiovascular diseases and psychiatry. 4. At a sociomedicinal level differences in the perception and description of symptoms as well as coping with diseases have to be considered for treatment strategies. In many cases, biological and social aspects cannot even be distinguished, since basic research in this filed is lacking. The fact, that many diseases and drugs which appear primarily to be sex-neutral (like myocardial infarction), exhibit sex-specific differences. Despite these pitfalls some recommendations for clinical practitioners are given, were and when gender should be taken into account for a drug prescription or monitoring of drug therapy.
