Management der Perforationen im Wandel (Verhütung und Behandlung)

Zusammenfassung

Mit der Verbreitung der diagnostischen und therapeutischen Endoskopie haben die damit verbundenen instrumentellen Verletzungen zugenommen. Die Verhütung von Perforationen setzt neben Beherrschung der Untersuchungs- und Behandlungstechniken Kenntnisse über die Schwach- bzw. Prädilektionsstellen an den Organen voraus. Management der endoskopischen Komplikationen ist Bestandteil des initialen ärztlichen Auftrages und gehört damit auch zur Verantwortung des Endoskopikers. Sofortige Erkennung und umgehende chirurgische Reparation bei Versagen der konservativen und endoskopischen Behandlungsversuche sind die wesentlichen Maßnahmen zur Schadenbegrenzung.

Abstract

With the wide-spread use of diagnostic and therapeutic endoscopy the number of procedure-related complications has accordingly increased. Prevention of perforation requires competence in practising endoscopic techniques and also knowledge about weak parts and locations at the gastrointestinal tract that are prawn to such complication. Management of endoscopic complications is part of the initial treatment and belongs therefore also to the responsibility of the endoscopist. Immediate recognition and surgical repair of perforation in case of failed conservative and endoscopic therapeutic trials are essential for further uneventful outcome.
