Der Husten, der häufigste oder zweithäufigste Grund für einen Arztbesuch, bietet eine Reihe von diagnostischen Fallen: Der akute Husten, der am häufigsten vorkommt, wird in überwiegender Mehrzahl der Fälle durch einen viralen Infekt der oberen und gegebenenfalls der unteren Atemwege verursacht. Das Symptom Husten hält in der Regel drei bis vier (bis zu acht) Wochen - also viel länger, als die „Erkältung” an. Zeit genug, um unnötige teuere diagnostische Maßnahmen oder unwirksame antibiotische Therapien zu fordern (der Patient) oder zu veranlassen (der Arzt). Außer Anamnese und einer körperlichen Untersuchung ist im typischen Fall eines akuten Hustens keine weitere Diagnostik erforderlich. Der akute Erkältungshusten ist eine Domäne der Selbstmedikation.
Der chronische Husten (über acht Wochen und länger) ist dringend abklärungsbedürftig, um ein Bronchialkarzinom, eine Tuberkulose oder eine Lungenstauung auszuschließen. Diagnostischer Standard ist eine Röntgenaufnahme der Thoraxorgane und eine Lungenfunktionsprüfung. Falls die Ursache des chronischen Hustens trotz dieser Standarddiagnostik unklar bleibt, sollte man an eine (oft banale) chronische Erkrankung der oberen Atemwege, an einen Husten als Asthmaäquivalent oder an eine gastroösophageale Refluxkrankheit denken. Rauchen kann ebenfalls für einen chronisch persistierenden Husten verantwortlich sein. ACE-Hemmer verursachen oft chronischen Husten, daher ist ein Auslassversuch (umstellen z. B. auf einen AT2-Antagonisten) an erster Stelle, vor Einleitung der weiteren Diagnostik indiziert. Etwa 10 % aller Patienten mit chronisch persistierendem Husten leiden an einer erhöhten Sensitivität des Hustenreflexes, ohne erkennbare Ursache.
The diagnostic of cough, one of the most important causes for consultations in primary care has several fallacies: The very frequent acute cough is mainly caused by an acute viral infection of the upper and or lower airway. The cough can last three- to four (up to eight) weeks and deserves neither further diagnostic (except history and physical examination) nor antibiotic treatment, often required from patients and prescribed by doctors. The acute cough is a domain of the OTC (over the counter) medications.
In contrast, chronic cough (over 8 weeks lasting) requires immediate scrutiny, i.e. chest x-ray and lung function test (beyond history and physical exam the second step of standard diagnostic) since chronic cough could be the first presenting symptom of severe conditions, such as bronchogenic carcinoma, tuberculosis or left heart failure. In some cases, despite of careful standard diagnostic procedure the cause of chronic cough remains unclear. If no angiotensin converting enzyme blocker is used, common chronic diseases of the upper airways (e.g. chronic rhinitis and sinusitis), cough type asthma and/or gastro-oesophageal reflux disease are the most frequent causes. Smokers have but are seldom complaining from chronic cough. However, up to 10 % of patients with chronic persistent cough are suffering from a hyperreactive cough reflex without any identifiable cause.
