Heterotopic Rectal Gastric Mucosa: A Rare Cause of Lower Gastrointestinal Bleeding in Children

Abstract

The authors report a very unusual case of intermittent and painful rectal bleeding occurring in a two-year-old female. At proctosigmoidoscopy, a bluish mucosal fold was found in the rectal ampulla. Histological examination showed the presence of heterotopic gastric mucosa islands intermingled with normal rectal mucosa. Surgical transanal excision of the lesion was undertaken due to the persistence of rectal bleeding with anemia, notwithstanding a three-month course of pharmacological therapy with a proton pump inhibitor. At a one-year follow-up, the child is well and symptom-free. The international pediatric literature was reviewed.

Hétérotopie gastrique dans la muqueuse rectale une cause rare d'hémorragie basse chez l'enfant

Les auteurs rapportent une très inhabituelle cause de saignements intermittents rectaux survenant chez une fille de 2 ans. Lors d'une procto-sigmoïdoscopie, un repli muqueux était retrouvé au niveau de l'ampoule rectale. L'étude histologique montrait la présence d'une hétérotopie de muqueuse gastrique avec des zones de muqueuses rectales normales. La chirurgie trans-anale pour exciser cette lésion a été réalisée devant la persistance des hémorragies rectales avec anémie ne réagissant pas après 3 mois de traitement par un inhibiteur de la pompe à protons. Après un mois de suivi, l'enfant est bien et sans symptôme. La littérature pédiatrique internationale a été revue.

Mucosa gástrica heterotópica rectal: una causa rara de hemorragia gastrointestinal baja en niños. Presentación de un caso y revisión de la literatura

Los autores presentan un caso muy raro de hemorragia rectal intermitente y dolorosa en una niña de 2 años. En la proctosigmoidoscopia se encontró un pliegue mucoso violáceo en la ampolla rectal. El examen histológico mostró la presencia de mucosa gástrica heterotópica alternada con mucosa rectal normal. Se llevó a cabo la extirpación quirúrgica transanal de la lesión debido a la persistencia de la hemorragia rectal con anemia tras 3 meses de tratamiento con inhibidores de la bomba de protones. Un año más tarde la niña estaba sin síntomas. Se revisa la literatura pediátrica internacional.

Heterotope Magenmukosa: Eine seltene Ursache von Blutungen des unteren Gastrointestinaltrakts. Fallbericht und Überblick über die Literatur

Es wird ein Fall einer sehr ungewöhnlichen, schmerzhaften intermittierenden, rektalen Blutung bei einem 2 Jahre alten Mädchen vorgestellt. Bei der Proktosigmoidoskopie fand sich eine bläuliche Schleimhautfalte in der rektalen Ampulle. Die histologische Untersuchung der Schleimhaut ergab ektopes Magengewebe mit dazwischen gelegener, normaler rektaler Schleimhaut. Eine transanale Exzision des Bezirkes wurde vorgenommen, nachdem es zu rezidivierenden Blutungen ungeachtet einer dreimonatigen Therapie mit einem Protonenpumpeninhibitor gekommen war. Im Nachuntersuchungszeitraum von einem Jahr ist das Kind jetzt beschwerde- und symptomfrei. Die entsprechende pädiatrische Literatur wird vorgestellt.
