Microbial Flora Alterations in Jejunum and Colon after Chemical Bowel Preparation before Kasai Hepatoportojejunostomy

Abstract

Aim: There is little reported evidence on the effect of preoperative chemical bowel preparation by the administration of oral antibiotics in patients with biliary atresia (BA). This study was designed to examine and compare the alterations of microbial flora in feces cultures before and after the administration of oral antibiotics, and to examine fluid cultures from the tip of the jejunal limb at bile duct reconstruction by Kasai hepatoportoenterostomy (HPJ), and to evaluate the effects of preoperative oral antibiotics on postoperative cholangitis. Material and Methods: Between January 2003 and July 2005, 6 infants with BA underwent surgical correction, and were treated preoperatively with polymyxin B (80 000 U/kg/day in 3 divided doses for 2 days) and metronidazole (10 mg/kg/day in 2 divided doses for 2 days). Quantitative feces cultures, both before administration of oral antibiotics and at Kasai HPJ, were obtained in patients with BA. Furthermore, fluid cultures from the tip of the jejunal limb at bile duct reconstruction by Kasai HPJ were obtained to examine alterations of bacteria and fungi. Results: Bacterial colonization of the tip of the jejunal limb did not occur at the Kasai HPJ. The most commonly encountered organisms were Enterococcus and intestinal bacterial flora. Fungus was not detected in the feces before or after the administration of oral antibiotics. Candida was detected in the tip of the jejunal limb after Kasai HPJ in only one patient. Conclusions: The authors propose that colonic-type flora are generally found in the feces before and after the administration of oral antibiotics, and no bacteria are detected in the bilioenteric conduits.

Modification de la flore micro-biologique dans le jéjunum et le colon apres preparation de l'intestin avant hépato-porto-jéjunostomie de Kasai

Buts: Il y a peu de publications sur l'effet de la préparation par des antibiotiques donnés par voie orale chez les patients avec une atrésie des voies biliaires. Cette étude a été réalisée pour comparer les altérations de la flore microbienne dans les selles et dans les liquides de culture prélevés à l'extrémité de l'anse jéjunale utilisée pour la reconstruction selon Kasai (HPJ) avant et après l'administration d'antibiotiques par voie orale et pour évaluer les effets de cette antibiothérapie sur le risque de cholangite post-opératoire. Matériel et Méthodes: De janvier 2003 à juillet 2005, six enfants avec une atrésie des voies biliaires ont subi une correction chirurgicale et ont été traités en pré-opératoire avec de la polymyxin B (80 000 U/kg/jour en 3 fois pendant 2 jours) et métronidazole (10 mg/kg/jour en deux prises pendant 2 jours). Les cultures des selles, à la fois avant et après Kasai, étaient obtenues chez les patients porteurs d'une atrésie des voies biliaires. Par ailleurs, des cultures du liquide prélevées au sommet de l'anse jéjunale au moment de la reconstruction selon Kasai étaient obtenues et examinées pour rechercher des modifications du contenu bactérien ou fongique. Résultats: La colonisation bactérienne de l'extrémité de l'anse jéjunale ne survient pas au moment du Kasai. Le micro-organisme le plus souvent rencontré est un entérocoque et une flore bactérienne intestinale. Aucune prolifération fungique n'est détectée dans les selles avant et après l'administration d'antibiotiques par voie orale. Le candida est détecté au sommet de l'anse jéjunale au moment du Kasai chez un seul patient. Conclusion: Les auteurs proposent que la flore colique typique est généralement trouvée dans les selles avant et après l'administration d'antibiotiques par voie orale et aucune bactérie n'est détectée dans le conduit bilio-intestinal.

Anomalias de la flora microbiana en el yeyundo y colon tras preparación química del intestino antes de la hepatoportoyeyunostomía de Kasai

Objetivo: Se conoce mal el efecto de la preparación química preoperatoria con antibióticos orales en pacientes con atresia biliar (BA). Este estudio pretende examinar y comparar las alteraciones de la flora bacteriana en cultivos fecales y del cabo distal del asa yeyunal usada para la reconstrucción del conducto biliar en la hepatoportoyeyunostomía de Kasai (HPJ) antes y después de la administración de antibióticos orales y evaluar los efectos de éstos sobre la colangitis postoperatoria. Material y Métodos: Entre enero de 2003 y julio de 2005 se operaron 6 pacientes con BA que se trataron preoperatoriamente con polimixina B (80 000 U/kg/día en 3 dosis durante 2 días) y Metronidazol (10 mg/kg/día en 2 dosis durante 2 días). Se obtuvieron cultivos fecales cuantitativos antes y después de la administración de antibióticos orales y durante la HPJ de Kasai. Se obtuvieron además cultivos de la punta del asa yeyunal antes de la anastomosis para examinar bacterias y hongos. Resultados: No hubo colonización bacteriana del extremo del asa yeyunal en la HPJ de Kasai. Los organismos más frecuentemente encontrados fueron enterococcus y flora bacteriana intestinal. No se detectaron hongos en las heces antes o después de la administración de antibióticos orales. Se detectó candida en el extremo del asa en solamente un paciente. Conclusión: Los autores proponen que la flora colónica habitual se encuentra en las heces antes y después de la administración de antibióticos mientras que no se detectan bacterias en los conductos bilio-entéricos.

Veränderungen der Darmflora im Jejunum und Kolon durch eine präoperative Dekontamination des Darmes vor einer Hepatoporto-Jejunostomie nach Kasai

Zielsetzung: Es wurde bisher wenig über die Wirksamkeit einer präoperativen Dekontamination des Darmes durch orale Antibiotikagaben vor Operation bei Gallengangsatresie berichtet. Die vorliegende Studie wurde deshalb durchgeführt, um Veränderungen der Darmflora im Jejunum während der Kasai'schen Operation im Vergleich zur Darmflora vor Dekontamination des Darmes zu untersuchen. Besonderes Augenmerk galt dabei dem Auftreten einer postoperativen Cholangitis. Material und Methode: Zwischen Januar 2003 und Juli 2005 wurden 6 Kinder nach Kasai operiert und präoperativ mit Polymyxin B (80 000 U/Tag in drei Dosen über 2 Tage) sowie Metronidazol (10 mg/kg/Tag verteilt auf zwei Dosen über 2 Tage) behandelt. Quantitative Stuhlkulturen wurden vor Antibiotikagabe und während der Hepatoportoenterostomie durchgeführt. Darüber hinaus wurden Flüssigkeitsproben aus dem zu anastomosierenden Jejunumschenkel intraoperativ entnommen und auf Bakterien und Pilze untersucht. Ergebnisse: Eine bakterielle Besiedlung der Jejunalschlinge konnte nicht nachgewiesen werden. Der insgesamt am häufigsten nachweisbare Keim war Enterokokkus. Pilze konnten im Stuhl weder vor noch nach Antibiotikagabe nachgewiesen werden. Nur bei einem Patienten fand sich Candida in der ausgeschalten Jejunumschlinge. Schlussfolgerungen: Typische Dickdarmflora wird im Stuhl generell vor und nach der Gabe oraler Antibiotika gefunden. Im Bereich der bilio-enteralen Anastomose konnten keine Bakterien nachgewiesen werden.
