Hybridtherapie bei Aneurysma verum des Aortenbogens

Einführung
Die chirurgische Therapie eines Aortenbogenaneurysmas im hypothermen Kreislaufstillstand hat eine Mortalitätsrate von 2 - 9 % [1] [2] [3] [4] [5]. Ein offener Eingriff mit kardiopulmonalem Bypass ist speziell bei multimorbiden Patienten mit den Gefahren einer systemischen Inflammation und parenchymatösen Ischämie verbunden. Nach hypothermem Kreislaufstillstand werden bei 3 - 13 % der Patienten permanente neurologische Defekte beobachtet [1] [2] [3] [4] [5] [6]. Alternativ zur offenen Resektion bietet sich die endovaskuläre Stentgraft-Implantation als therapeutische Option in der Behandlung von Aneurysmen der thorakalen Aorta an [7] [8] [9] [10] [11]. Grundsätzlich ist aufgrund der hohen Flexibilität der neuen Generation von Stentgrafts eine Platzierung im Aortenbogen möglich, wobei allerdings die Anatomie der Halsgefäße die technischen Grenzen aufzeigt [12]. In einem innovativen Ansatz wird eine Alternative zur klassischen Aortenchirurgie bei multimorbiden Patienten mit hohem perioperativen Risiko vorgestellt.
