Tonische Halbseitenanfälle nach Elektrotrauma

Zusammenfassung

Der Fall eines 45-jährigen Patienten wird vorgestellt, der nach einem Stromunfall typische schmerzhafte tonische Halbseitenanfälle entwickelte. Da der Strom durch die Extremitäten einer Körperseite floss und der Patient nicht bewusstlos wurde, nehmen wir einen spinalen Ursprung an. Die Anfälle sprachen gut auf eine Kombinationstherapie mit Carbamazepin und Gabapentin an. Der Fall lässt vermuten, dass tonische Halbseitenanfälle, bisher meist als tonische Hirnstammanfälle bei MS beobachtet, im Prinzip in jedem Abschnitt der Pyramidenbahn entstehen können.

Abstract

The case of a 45-year-old patient who suffered an electric injury and subsequently developed typical unilateral tonic spasms is presented. Sience the current passed through the extremities of one side and the patient did not lose consciousness, we hypothesised a spinal origin. The attacks responded well to a combination therapy of carbamazepine and gabapentin. The case suggests that tonic spasms, usually observed as tonic brain stem attacks in MS, may originate from any segment of the pyramidal tract.
