(E)US-Elastografie: Heutiger Stand und Perspektiven

Zusammenfassung

Die (endo)sonografische Elastografie ist ein neues Verfahren zur Beschreibung der mechanischen Gewebeeigenschaften in einem Messfenster, das wie bei der Duplexsonografie in das B-Bild hineingelegt wird. Im Messfenster werden die relativen Gewebehärten farblich dargestellt, ohne dass eine absolute Skalierung der Elastizitätsangabe erfolgt. Mit kommerziell erhältlichen Systemen ist die Elastografie zur Zeit transkutan nur mit Linearschallköpfen, endokavitär mit starren Sonden sowie endosonografisch mit flexiblen Echoendoskopen durchführbar. Linearschallköpfe und starre endokavitäre Schallköpfe werden zur Druckausübung auf das Gewebe benutzt. Aus der resultierenden Gewebeverformung wird der Elastizitätsmodulus berechnet. Endosonografisch werden Gewebedeformierungen infolge arterieller Pulsationen, der Herzbewegungen oder von Atemexkursionen der Berechnung zugrunde gelegt. Erste Anwendungen im Bereich der Diagnostik von Tumoren der Mamma, der Prostata, der Cervix uteri und der Schilddrüse zeigen, dass die Elastografie technisch funktioniert und differenzialdiagnostisch erfolgreich genutzt werden kann. Endosonografisch wurde die Realtime-Elastografie bislang zur Charakterisierung von Lymphknoten und Pankreasgewebe eingesetzt. Die Dignität von Lymphknoten kann mit einer Treffsicherheit von 85 - 90 % vorhergesagt werden, sodass im Einzelfall geeignete Lymphknoten für die Feinnadelbiopsie gezielter ausgesucht werden können. Die elastografischen Muster von Malignomen des Pankreas sind sehr einheitlich und vom gesunden Pankreas sicher zu unterscheiden, zeigen aber aufgrund des ähnlichen biomechanischen Aufbaus eine breite Überlappung mit den Elastogrammen bei der chronischen Pankreatitis. Daher wird die Methode vermutlich nicht für die Frühdiagnostik des Pankreaskarzinoms in der chronischen Pankreatitis genutzt werden können. Zusammenfassend ist die (endo)sonografische Elastografie eine faszinierende neue Methode, deren Einsatzgebiete und klinischer Stellenwert aber noch durch prospektive Studien zu definieren sein werden.

Abstract

(Endo)sonographic real-time elastography is a new method to describe the mechanical properties of tissue. Similar to colour flow Doppler ultrasonography, a region of interest is defined. The relative stiffness of the tissues within this area is described by colours superimposing on the B-mode image. Real-time elastography can be performed with linear scanners for transcutaneous use, rigid endocavitary probes and with flexible echoendoscopes. The probes can be used to compress the tissue. The elasticity modulus is calculated from the resulting deformation of the tissue. In endoscopic ultrasound, arterial and cardiac pulsations or respiratory movements cause the deformation of the tissue that is used for the calculation. Several studies have demonstrated that real-time elastography is feasible and improves the diagnostic accuracy for tumours of the breast, the prostate, the cervix, and the thyroid gland. Endosonographic elastography has been employed in the examination of lymph nodes and the pancreas. For the differentation between benign and malignant lymph nodes, the accuracy is reported to be 85 % to 90 %. Therefore, the method seems to be useful to select lymph nodes suitable for biopsy. The elastographic pattern of malignant tumours of the pancreas is different from that of the normal pancreas, but similar to that of chronic pancreatitis due to the same biomechanical architecture. Therefore, the early diagnosis of cancer within chronic pancreatitis will probably not be improved by elastography. In summary, (endo)sonographic real-time elastography is a promising new method. Nevertheless, prospective studies are needed to define useful applications and the clinical significance of the method.
