Pulmonale Komplikationen nach hämatopoetischer Stammzelltransplantation – eine Herausforderung für die Intensivpflege

Zusammenfassung

Die Transplantation hämatopoetischer Stammzellen ist heute bei einer Vielzahl von hämatoonkologischen Grunderkrankungen Therapie der ersten Wahl. Allein in Europa wurden im Jahr 2006 etwa 25 000 Knochenmark- bzw. Blutstammzelltransplantationen durchgeführt. Die besondere Relevanz der SZT sowohl für die Intensivmedizin als auch für die Intensivpflege ergibt sich aus der Tatsache, dass bis zu 40 % aller stammzelltransplantierten Patienten wegen des Auftretens schwerer Komplikationen der Intensivtherapie bedürfen. An erster Stelle der Indikationen zur Intensivtherapie bei Patienten nach Stammzelltransplantation stehen dabei Formen der respiratorischen Insuffizienz. In diesem Beitrag werden deshalb die verschiedenen pulmonalen Komplikationen nach ihrer Genese und ihrem zeitlichen Auftreten dargestellt. Da der stammzelltransplantierte Patient beim Auftreten pulmonaler Komplikationen das Behandlungsteam vor große Herausforderungen stellt, wird speziell auf die pflegerelevanten Besonderheiten näher eingegangen. Weil der stammzelltransplantierte Patient immer auch als ein kritisch Kranker aufzufassen ist, ist eine inhaltliche Durchdringung der mit diesen Patienten verbundenen Problemkonstellationen für die Intensivpflege auch in Zukunft von besonderer Bedeutung. Der folgende Artikel will dazu einen Beitrag leisten.
