Zusammenfassung
Wir beschreiben eine Patientin mit einer ungewöhnlichen klinischen Präsentation einer Neurotuberkulose. Bei typischen Befunden im Liquor und multiplen zerebralen Tuberkulomen, jedoch fehlender meningealer Reaktion im MRT des Kopfes wurde die immunkompetente 65-jährige Patientin ausschließlich durch ein akut aufgetretenes hirnorganisches Psychosyndrom klinisch auffällig. Die Diagnose wurde durch die Liquordiagnostik gestellt und durch PCR und durch mikroskopischen Erregernachweis bestätigt. Der Fall wird anhand der vorliegenden Literatur diskutiert.
Abstract
We present a case of neurotuberculosis with atypical clinical features. The 65-year-old immunocompetent female presented with an acute organic brain syndrome but without meningeal irritation or focal signs. We detected multiple small tuberculomas, but no meningeal enhancement in MRI. The CSF analysis was typical for tuberculous meningitis. The patient recovered completely with anti-tuberculous treatment. The case is discussed in the context of the current literature.
