Heparininduzierte Thrombozytopenie Typ II und alternative Antikoagulation bei CVVH

Zusammenfassung

Die Antikoagulation intensivmedizinischer Patienten mit heparininduzierter Thrombyzytopenie (HIT) und Nierenversagen stellt eine Herausforderung dar. Bei Patienten ohne Leberfunktionsstörung wird Argatroban bevorzugt. Bei zusätzlicher Leberdysfunktion stellt Danaparoid eine Alternative dar. Nachteilig ist neben der langen Halbwertszeit die erschwerte Elimination bei Überdosierung. Weiterhin steht Lepirudin zur Verfügung, das mit erhöhter Blutungsgefahr einhergeht und durch die "Ecarin clotting time" kontrolliert wird. Erfahrungen mit Antikoaglanzien wie Bivalirudin, Fondparinux und Prostaglandinen sind bisher begrenzt und zukünftige Studien sollten den Stellenwert dieser Substanzen bei HIT und Nierenersatztherapie bewerten. Auch ob der kombinierte Einsatz von Antikoagulanzien zur Therapie der HIT und Zitrat die Filterstandzeiten signifikant verlängern kann, sollten zukünftigen Untersuchungen überprüfen.

Abstract

The decision for an anticoagulant for renal replacement therapy (RRT) in patients with acute renal failure and heparin–induced thrombocytopenia (HIT) has to be made carefully. Based on results from the literature argatroban is favoured in patients without hepatic dysfunction, referring to its short halftime and easy feasable monitoring. In the case of coexsisting hepatic disorder, danaparoid provides a safe alternative therapy. However, long halftime and the difficult elimination of the substance are unfavourable. Lepirudin represents another possible anticoagulant therapy. Bleeding complications and monitoring of the ecarin clotting time imposes limitations. Experiences with bivalirudin, fondaparinux and prostaglandines are limited and future trials will have to determine the significance of their application in RRT in HIT patients. Furthermore it has to be proven whether the combination of alternative anticoagulants with citrate prolongates circuit halftime of CVVH.

• Der HIT Typ II ist – bei einer niedrigen Inzidenz von 0,5 % auf Intensivstationen und einer Mortalität von 25 % – eine nicht unerhebliche Bedeutung beizumessen.

• Die Sepsis als Grunderkrankung bei einem großen Anteil der Intensivpatienten geht mit einer prokoagulatorischen Gerinnungssituation einher.

• Das Risiko thrombembolischer Komplikation steigt bei einer HIT Typ II sprunghaft an.

• Als klinische Manifestation sind plötzlich auftretende Lungenembolien, Schlaganfälle, Myokardinfarkte sowie venöse und arterielle Gefäßverschlüsse der Extremitäten zu nennen.

• Eine überschießende Gerinnungsneigung im Dialysefilter sollte an die Diagnose einer HIT Typ II denken lassen.

• Bei bestehendem Verdacht auf eine HIT sollte nach Applikationsabbruch eine HIT–Serologie veranlasst und eine alternative Antikoagulation begonnen werden.

• Von den aktuell erhältlichen alternativen Antikoagulanzien ist Argatroban das Medikament mit der kürzesten Halbwertszeit, das kontinuierlich unter Überwachung der aPTT appliziert werden kann.

• Vorsicht ist bei gleichzeitiger Leberdysfunktion geboten: Es sollte umgehend eine Dosisreduktion oder Umstellung auf ein anderes Antikoagulans erfolgen.

• Danaparoid, Hirudine und Fondaparinux weisen aufgrund ihrer renalen Elimination längere Plasmahalbwertszeiten auf. Sie dürfen nur unter strengem Monitoring und Anpassung der Dosierung eingesetzt werden.

• Bei Lepirudin sollte die Wirkung über eine ECT–Messung gemessen werden. Nur dann ist die Anwendung dieses Antikoagulans zu empfehlen.

• Die Rate der Blutungskomplikationen unter Applikation von Lepirudin steigt durch die Ausbildung von Antikörpern, welche die Halbwertszeit zusätzlich verlängern.

• Für Danaparoid sowie die Hirudine und Fondaparinux existiert kein Antidot. Versuche zur Elimination mittels CVVH oder Plasmapherese können Blutungskomplikationen nur teilweise verhindern.
