Magnetic resonance imaging (MRI) is ideally suited to evaluating injuries of the foot and ankle. The spatial and contrast resolution attainable with MRI, in combination with its multiplanar imaging capabilities, allows accurate detection of bone and soft tissue injury and clear delineation of complex anatomy. Selection of imaging planes and pulse sequences should be tailored to the clinical question at hand. MRI is the preferred imaging modality for detection of occult fracture and for evaluation of tendon or ligament injury. In patients with osteochondritis dissecans, it provides accurate staging and may be used to guide treatment.
