Chin ptosis is either iatrogenic or developmental in origin. This article will focus on the evaluation and treatment of the latter. Developmental causes of chin ptosis can be subdivided into both static and dynamic deformities. Static chin ptosis can result from the inferior migration of the mentalis muscle origin, a deep submental crease secondary to gravitational chin pad droop, or related to submental fat removal. In all cases the soft tissue chin pad extends below the level of the menton. Dynamic chin ptosis denotes a condition whereby the chin pad is effaced against bone and downwardly displaced during smiling. Precise anatomic diagnosis of developmental chin ptosis is essential for its proper correction. This article will review the clinically relevant chin anatomy, classify the causes of developmental chin ptosis, and recommend treatments based on the underlying pathology.
