ABSTRACT

The introduction of the anterior facial aesthetic techniques, which require access through the lower lid, has vastly expanded. Complications that occur with these techniques are directly associated with the severity of lower lid malpositioning. Having an “all-weather-adjustable canthopexy” can solve this problem by allowing surgeons to access the lateral canthal tendon area through a hidden brow incision, which in turn eliminates further eyelid skin incisions. With an incision in the brow, using a subperiosteal and transconjunctival approach, lower lid and lateral canthal positioning is easier and more predictable. In difficult asymmetrical cases, adjusting suture positioning and canthal tension through the eyebrow position can improve the success of this type of procedure.
