Behandlung des primären Spontanpneumothorax beim Jugendlichen – State of the art

Der primäre Spontanpneumothorax (PSP) beschreibt eine Ansammlung von Luft im intrapleuralen Raum, welcher zum teilweisen oder vollständigen Kollaps der Lunge führen kann. Dieser tritt am häufigsten bei Jugendlichen über 15 Jahre, (männlich > weiblich) mit asthenischem Habitus und großer Körperlänge auf. In dieser Übersichtsarbeit werden aktuelle Kontroversen bezüglich der optimalen Therapie (konservativ vs. Pleuradrainage vs. Operation, Management von Rezidiven) sowie der Stellenwert der thorakalen Computertomografie diskutiert.
ABSTRACT
Primary spontaneous pneumothorax is described as an accumulation of air in the intrapleural space that can cause partial or total collapse of the ipsilateral lung. This occurs most frequently in adolescents over 15 years of age (males > females) with asthenic body habitus and high body size. Here we discuss current controversies regarding the optimal therapy (conservative vs. chest tube vs. surgery, management of recurrent disease), and also the value of chest CT.
