Gastrointestinale Endoskopie im postoperativen Komplikationsmanagement

Die gastrointestinale Endoskopie hat sich in den letzten Jahren als essenzielles Instrument im Management postoperativer Komplikationen etabliert. Sie ermöglicht nicht nur eine präzise Diagnostik, sondern auch eine minimalinvasive Therapie, die in vielen Fällen eine erneute Operation vermeiden kann. Dieser Artikel beleuchtet die Rolle der gastrointestinalen Endoskopie im postoperativen Management von Komplikationen wie Ischämien, Blutungen und Anastomoseninsuffizienzen.

Operationsindikation, technische Durchführung der OP sowie das Management postoperativer Komplikationen sind ein fester Komplex. Ein Qualitätsverlust in einem dieser 3 Abschnitte oder ein partielles Outsourcing führt zu einer Verschlechterung des Gesamtergebnisses.

Die frühzeitige Diagnose ist ein wichtiger Prognosefaktor im postoperativen Komplikationsmanagement.

Bei klinischen Auffälligkeiten im postoperativen Verlauf nach chirurgischem Eingriff ist immer primär an eine chirurgische Komplikation zu denken.

In Bezug auf Ischämie, Blutung oder Anastomoseninsuffizienz kann die endoskopische Abklärung eine rasche Diagnose und oft auch eine suffiziente Therapie anbieten.

Eine solche endoskopische Intervention erfordert viel Sorgfalt und Expertise bezüglich der Beurteilung des operativen Situs und der Konzeption der besten Therapieoption (Endoskopie versus Radiologie versus operative Revision); die Methodenwahl hängt von vielen Faktoren ab. Es gibt keine generell überlegenen Methoden oder verbindliche Goldstandards.

Bei besonders schwierigen Fällen hat sich die Kombinationsbehandlung verschiedener endoskopischer Methoden bewährt.
