Zusammenfassung

Beim Morbus Perthes, der juvenilen Hüftkopfnekrose, kommt es zur Verminderung oder Unterbrechung der Blutversorgung der Epiphyse des Femurkopfes. Über die Ätiologie besteht bislang keine Einigkeit, systemische wie lokale Faktoren werden diskutiert. Die Erkrankung verläuft in verschiedenen Stadien. Die Diagnose kann in der Regel mittels konventioneller Röntgenaufnahmen gestellt werden. Das heute allgemein anerkannte therapeutische Prinzip besteht in dem Erhalt des Containments. Dies kann in Abhängigkeit von dem Schweregrad durch konservative Maßnahmen, häufig aber nur durch operative Eingriffe, erzielt werden. Die Langzeitprognose ist im Wesentlichen abhängig von der Sphärizität und Kongruenz zwischen Hüftkopf und Pfanne nach Ausheilung.

Summary

Perthes disease is characterized by a temporarily reduced blood supply of the femoral epiphysis resulting in femoral head necrosis. Concerning aetiology many systemic and locals factors are discussed. The course of the disease is characterized by defined stages. The diagnosis is usually made by X-ray. The widely accepted treatment concept is described by the „containment“ principle. Depending on the severity of the disease this may be achieved by conservative and/or surgical procedures. The longterm prognosis is dependent on the sphericity and congruency of the femoral head and the acetabulum in the final stage.
