Zusammenfassung
Einleitung: Bei steigender Inzidenz des Barrettkarzinoms wäre die Videokapselendoskopie (VKE) ein denkbares Screeningverfahren. Die vorliegende Arbeit evaluiert daher den diagnostischen Wert der Kapselendoskopie im Ösophagus. Methode: Die Videokapselaufnahmen von 30 Patienten wurden von einem hinsichtlich des Gastroskopiebefundes verblindeten Untersucher beurteilt und das Ergebnis mit dem schriftlichen Gastroskopiebefund verglichen. Die konventionellen Gastroskopien hatten einen mittleren Abstand von zwei Wochen zur VKE. Ergebnisse: Die VKE zeichnete durchschnittlich 10 Einzelbilder im Ösophagus auf, gut beurteilbar waren jedoch nur ein Viertel. Bei 13 Patienten zeigten sich 16 pathologische Befunde. Allerdings konnten mehr als ein Drittel der in der VKE vermuteten Befunde gastroskopisch nicht bestätigt werden. Umgekehrt wurden bei 10 Patienten 13 gastroskopisch nachgewiesene Befunde in der VKE nicht dargestellt. Schlussfolgerungen: Die VKE in der gegenwärtigen Form eignet sich nicht für die Beurteilung des Ösophagus. Durch Modifikation von Technik und Einnahmemodalitäten könnte jedoch die Beurteilbarkeit verbessert werden.
Abstract
Background: In respect to the increasing incidence of Barrett's carcinoma Video-capsule-endoscopy (VCE) could be a possible screening method. The present analysis evaluates the diagnostic yield of VCE in the oesophagus. Method: The VCE pictures of 30 patients were investigated by an endoscopist, which was blinded to the results of the standard gastroscopy, performed within a mean intervall of two weeks. Results of VCE and gastroscopy were compared. Results: On average 10 pictures of the oesophagus were recorded, but only one fourth of the pictures had a sufficient quality. VCE showed 16 pathological oesophageal findings in 13 patients. But more than one third of the VCE findings were not proved by gastroscopy. Additionally, VCE did not show 13 gastroscopic findings in 10 patients. Conclusions: At present, VCE is not suitable for investigation of the oesophagus. After modification of technical features and ingestion modalities the diagnostic value might be improved.
