Einmal Stiefkind, immer Stiefkind?

Besteht Diarrhö als wesentliches Symptom, empfiehlt die jüngst aktualisierte S3-Leitlinie Reizdarmsyndrom [1] (im Folgenden RDS-Update) grundsätzlich eine umfassende diagnostische Abklärung mit dem Hinweis: “Eine besondere, teilweise noch unterschätzte Bedeutung kommt hierbei dem Gallensäurenverlustsyndrom zu”. Die damit geweckte Erwartung, dass mit diesem Update neben dem Gallensäurenverlustsyndrom nach Ileumresektion oder bei Ileitis Crohn jetzt auch die primäre chologene Diarrhö und assoziative Formen wie die chologene Diarrhö nach Cholezystektomie Eingang finden, wird aber enttäuscht.
