Im ambulanten Bereich wird die Hauptversorgung des Erkrankten von seiner Familie durchgeführt, unterstützt durch den Hausarzt und bei Bedarf durch ambulante Pflege-, Palliativ- und Hospizdienste. Stationäre Einrichtungen stehen im Hintergrund, falls die ambulante Versorgung entgleist. Es zeigte sich in der Vergangenheit, dass bei schweren palliativen Symptombildern das herkömmliche Versorgungssystem einschließlich der professionellen Dienste oft an die Grenzen seiner Möglichkeiten gelangt ist und Patienten häufig in der Finalphase in das Krankenhaus eingewiesen werden, obwohl sie den Wunsch haben, zu Hause zu bleiben. Mit zunehmender Verbreitung qualifizierter ambulanter palliativmedizinischer und -pflegerischer Dienste wird es künftig möglich sein, dem Patientenwunsch nach Versorgung in den eigenen 4 Wänden auch bei schwersten Symptomkonstellationen zu entsprechen.
In the outpatient setting the main source of care for a patient comes from his/her family, with support by the family doctor and, when necessary, outpatient nursing, palliative and hospice services. Hospital facilities remain in the background for the case that outpatient care fails. In the past it was found for severe palliative symptom situations that the usual health-care system including professional services often reached its limits and that the patient had to be admitted to hospital for the final stages even though he/she had expressed the wish to remain at home until the end. With the increasing availability of qualified outpatient palliative medical and nursing services it should in future be possible to fulfill the patient's wish for care in his/her own home even in cases of the most severe symptom constellations.
