Amplitudenkodierte Farbdopplersonographie im Kindesalter

Zusammenfassung
Ziel: Die Möglichkeiten der amplitudenkodierten Farbdopplersonographie (aFDS) im pädiatrischen Krankengut zu evaluieren und zu diskutieren. Methode: Anhand von 270 Patienten (Alter: Neugeborenenalter bis 16 Jahre) wurden verschiedenste aFDS Applikationen mit den Befunden der FDS sowie mit sonstigen Untersuchungsergebnissen aus Szintigraphie, Computertomographie, Magnetresonanztomographie, Angiographie, sowie operativen, labormedizinischen und klinischen Befunden korreliert. Ergebnisse: Die aFDS verbessert deutlich die Darstellung der insbesondere peripheren Gefäßarchitektur/Vaskularisation. Sie war bei 92,4 % der Patienten in diagnostischer Qualität durchführbar. Besonders interessante Anwendungen mit klinischer Relevanz sind: fokale Nierenerkrankung (Treffsicherheit 87,1 %), Differenzialdiagnose von Hodenerkrankungen (Treffsicherheit = 98 %), Lymphknotenveränderungen (Treffsicherheit = 92 %), Tumoren und Pseudotumoren, sowie sonstige Small Parts Fragestellungen. Dabei lieferte die aFDS in Abhängigkeit von der Applikationsregion einen diagnostischen Zugewinn bei 18,6 % bis 48 % der Patienten. Schlussfolgerungen: Die aFDS stellt als ergänzendes Instrumentarium zur konventionellen FDS und Duplex-Sonographie bei gezielter Fragestellung trotz der Artefaktanfälligkeit eine auch bei Kindern gut anwendbare Methode dar. Sie bereichert die sonographische Diagnostik und besitzt, insbesondere im Zusammenhang mit neuen Verarbeitungstechniken und Ultraschallkontrastverstärkern, noch weiteres Entwicklungspotenzial.
Amplitude Coded Colour Doppler Sonography in Paediatrics
Objective: The goal of this study was to evaluate the use of amplitude coded colour Doppler sonography (aCDS) in infants and children. Methods: We analysed our own experiences in 270 patients (mean age: 6.3 years, range: newborn -16 years) with various paediatric aCDS applications. The aCDS results of 460 individual investigations were correlated with conventional CDS results, clinical findings and the results of other imaging modalities. Additionally, relevant literature is reviewed and discussed. Results: ACDS is a valuable tool in paediatric sonography. It enriches the diagnostic field of ultrasound, particularly in regard to renal, abdominal, oncological and small part applications. ACDS could successfully be performed in 92.4 % of investigations. Overall accuracy was 86.6 % (in regard to the total number), sensitivity was 92 % (calculated for the number of successful investigations), additional diagnostic information was gained in 18.6 % to 48 % of patients - depending on the field of application. Conclusion: ACDS is applicable to the majority of infants and children, in spite of the risk of motion artefacts. It is a valuable addition to conventional paediatric ultrasound and colour Doppler sonography. In conjunction with new modalities like three-dimensional sonography, harmonic imaging and ultrasound contrast agents it promises to continuously widen sonographic diagnostic potential.
