Results of Nuss Procedures for the Correction of Pectus Excavatum

Abstract

Introduction: The principle of the Nuss procedure is remodulation of the chest using a steel bar inserted by means of two small skin incisions along the axillary lines. Purpose of the Study: The assessment of the outcome after treatment. Material and Methods: Between July 1998 and March 2003 66 children were operated on by Nuss technique to correct pectus excavatum in our department. Age ranged from 1 to 19 years (mean 11.8 years). The evaluation prior to surgery included chest X-ray and CT, pulmonology consultation with pulmonary function tests and cardiology consultation. We used a modified technique in which we inserted the supporting bar directly from the right side, with no thoracoscope, no sharp tools and no guiding tape. Intrapleural or epidural bupivacaine was used for postoperative pain control. Results: Duration of the operation was between 20 and 60 min. Complications included wound infections in six children, haemothorax in three and pneumonia in one. In 56 children the early cosmetic results were very good. In 5 children the excavation of the lower part of the sternum has persisted, in 5 children the excavation of the sternum was corrected but asymmetric thickening in the parasternal region persisted. In all cases the results were satisfactory for both the children and their parents. In 24 children the supporting bar was removed two years after the first operation. Postoperative courses were uneventful and the cosmetic results were good. In one child we observed a recurrence of the deformity six months after bar removal. Conclusions: The early results of the new operation for pectus excavatum are good in most children. The minimal invasiveness of this method suggests that this procedure is indicated in cases where the defects are only cosmetic.

Introduction: Le principe de l'intervention de Nuss est la correction du pectus excavatum en utilisant une barre métallique insérée par deux petites incisions cutanées sur les lignes axillaires. But: Apprécier les résultats de ce traitement. Matériel et Méthodes: Dans notre département, de Juillet 1998 à Mars 2003, 66 enfants étaient opérés par la technique de Nuss pour corriger un pectus excavatum. L'âge des patients était de 1 à 19 ans (moyenne 11,8 ans). L'évaluation des patients avant la chirurgie comportait une radiographie du thorax et un CT scan, une consultation de pneumologie avec des épreuves fonctionnelles respiratoires et une consultation de cardiologie. Nous avons utilisé une technique modifiée dans laquelle nous insérons la barre directement par le côté droit sans thoracoscopie, sans instrument tranchant et sans guide. De la bupivacaine intra pleurale et épidurale était utilisée pour le contrôle post-opératoire de la douleur. Résultats: La durée des opérations était de 20 à 60 minutes. Les complications étaient une infection pariétale chez six enfants, un hémothorax chez trois et une pneumonie chez un. Chez 56 enfants, le résultat esthétique était très bon. Chez 5 enfants, il persistait une excavation de la partie basse du sternum, chez 5 enfants l'excavation sternale était corrigée mais une asymétrie de la région para sternale persistait. Dans tous les cas, le résultat était satisfaisant à la fois pour les parents et les enfants. Chez 24 enfants, la barre était retirée deux ans après la première opération. La période postopératoire était sans problème et le résultat esthétique bon. Chez un enfant nous avons observé une récidive de la déformation six mois après l'ablation de la barre. Conclusion: Les résultats de cette nouvelle opération sur le pectus excavatum sont bons chez la plupart des enfants. La caractère mini invasif de cette technique permet de l'utiliser dans les cas où l'anomalie esthétique est isolée.

Introducción: El principio del procedimiento de Nuss es la remodelación del tórax usando una barra metálica insertada a través de dos pequeñas incisiones cutáneas en las líneas axilares. Objetivo: Evaluar los resultados del tratamiento. Material y Métodos: En nuestro Departamento se operaron con este procedimiento entre julio de 1998 y marzo del 2003 66 niños con pectus excavatum. Las edades oscilaron entre 1 y 19 años (media 11,8). La evaluación de los pacientes antes de la cirugía incluyó placa de tórax y TAC, consulta de Neumología, test de función pulmonar y consulta de cardiología. Usamos una técnica modificada en la cual insertamos la barra directamente desde el lado derecho sin toracoscopio, sin instrumentos punzantes y sin cinta guía. Se utilizó bupivacaina intrapleural o epidural para controlar el dolor postoperatorio. Resultados: El tiempo operatorio fue de 20 a 60 min. Hubo complicaciones como infección de la herida en 6 pacientes, hemotórax en 3 y neumonía en 1. En 56 niños el resultado cosmético precoz fue muy bueno. En 5 niños la excavación de la parte baja del esternón persistió y en otros 5 la excavación del esternón se corrigió pero el engrosamiento asimétrico de la región paraesternal persistió. En todos los casos los resultados fueron satisfactorios tanto para los niños como para los padres. En 24 niños la barra fue retirada dos años tras la operación. Los periodos postoperatorios transcurrieron sin incidentes y los resultados cosméticos fueron buenos. En un niño hemos observado la recidiva de la deformidad 6 meses tras la retirada de la barra. Conclusion: Los resultados precoces de la nueva operación para pectus excavatum son buenos en la mayoría de los niños. La mínima invasividad de este método permite indicar este procedimiento en casos con solo problemas cosméticos.

Einleitung: Das Prinzip des Nuss-Verfahrens ist eine Remodulation der Brustwand mithilfe eines Metallbügels, der mittels zweier kleiner Hautschnitte eingeführt wird. Zielsetzung: Ziel der Studie war die Bewertung der Behandlungsergebnisse. Material und Methoden: Zwischen Juli 1998 und März 2003 wurden in unserer Abteilung 66 Kinder mit dem Nuss-Verfahren zur Korrektur einer Pectus excavatum operiert. Das Alter der Kinder reichte von 1 bis 19 Jahren (Mittel 11,8 Jahre). Die Kinder wurden vor der Operation untersucht: es wurden eine Röntgenaufnahme und ein CT des Brustkorbs durchgeführt sowie pneumologische und kardiologische Abklärungen mit pneumologischen Funktionsprüfungen. Wir verwendeten eine modifizierte Technik nach Nuss, bei der der Metallbügel von der rechten Seite eingeführt wurde ohne thoraskopische Kontrolle, ohne scharfe Instrumente und ohne die Verwendung eines Führungsbands. Bupivacaine wurde für die Kontrolle postoperativer Schmerzen intrapleural oder epidural verabreicht. Ergebnisse: Die Operationsdauer betrug zwischen 20 und 60 Minuten. Als Komplikationen traten Wundinfektionen bei sechs Kindern, Hämothorax bei dreien und eine Lungenentzündung bei einem Kind auf. Bei 56 Kindern waren die frühen kosmetischen Ergebnisse sehr gut. Bei 5 Kindern bestand die Einziehung des unteren Brustbeins weiter fort, bei 5 Kindern konnte die Einziehung des Brustbeins korrigiert werden, aber es bestand weiterhin eine asymmetrische Verdickung in der Parasternalregion. Bei allen Kindern waren die Ergebnisse sowohl für die Kinder als auch für ihre Eltern zufriedenstellend. Bei 24 Kindern wurde der Bügel zwei Jahre nach der Erstoperation wieder entfernt. Der postoperative Verlauf verlief bei allen Kindern ohne Zwischenfälle und die kosmetischen Ergebnisse waren gut. Bei einem Kind konnten wir ein Wiederauftreten der Deformität sechs Monate nach Entfernung des Bügels beobachten. Schlussfolgerungen: Die ersten Ergebnisse des modifizierten Verfahrens zur Korrektur von Pectus excavatum sind bei den meisten Kindern gut.
